摘要
目的对比研究早发型重度子痫前期患者和晚发型重度子痫前期患者胎盘绒毛组织在体外6%氧浓度下培养,其培养液中代谢足迹的差异。方法本研究纳入2014年1月至2016年12月就诊于我院重度子痫前期患者44例,根据子痫类型分为早发型重度子痫前期组(早发组21例)和晚发型重度子痫前期组(晚发组23例),全部患者接受剖宫产终止妊娠治疗。于胎盘母体面取绒毛组织,在6%氧浓度下培养96h,采用高效液相质谱色谱仪检测培养液中代谢产物。对比两组患者谷氨酸、谷氨酰胺、色氨酸和犬尿氨酸的浓度差异。结果在6%氧浓度下培养的早发组和晚发组绒毛组织中,36种代谢产物出现显著性差异(P<0.05);早发型重度子痫前期患者的胎盘绒毛组织培养基中,犬尿氨酸的浓度显著高于晚发型重度子痫前期组(P<0.05)。结论通过组织代谢足迹分析技术,犬尿氨酸含量可以有效鉴别早发型和晚发型重度子痫前期,对预测重度子痫前期提供新的技术途径,值得临床推广应用。
        Objective:To compare the difference of the placental villus tissue of early onset severe pre eclampsia and late onset severe pre eclampsia in vitro under 6% oxygen concentration.Methods:This study included 44 cases of severe preeclampsia in our hospital from January 2014 to December 2016,according to the type of eclampsia,divided into early onset of severe preeclampsia group(early onset group,21 cases)and late onset severe preeclampsia group(late onset group,23 cases),all patients received cesarean section treatment.The villus tissue was taken from the placenta surface,and 96 h was cultured at 6% oxygen concentration.The metabolites in the culture medium were detected by hplc.The concentrations of glutamic acid,glutamine,tryptophan,and uric acid in the two groups were compared.Results:Cultured in 6% oxygen concentration of early onset and late onset groups in villi,there were significant differences between the 36 metabolites(P<0.05);Placental villus tissue culture in patients with early onset severe preeclampsia,the concentration of serum uric acid was significantly higher than that in the group of late onset severe preeclampsia(P<0.05).Conclusion:Through metabolic footprint analysis techniques,kynurenine content can effectively identify early and late onset severe preeclampsia,prediction of severe preeclampsia providing a new approach,it is worthy of clinical application.
