摘要
目的探究HPV DNA感染与宫颈癌相关的检查指标对宫颈癌的诊断价值,了解宫颈癌患者及慢性宫颈炎患者生殖道感染的病原菌分布。方法选择2015年7月—2017年5月来我院就诊的50例宫颈癌合并生殖道感染的患者作为观察组,选择同期来我院就诊的50例慢性宫颈炎的患者作为对照组。分析TCT、HPV DNA、SCCA、CA15-3、CA125、CYFRA21-1、HPV E6/E7 m RNA诊断宫颈癌的敏感性、准确性、阳性预测值和阴性预测值。并分析宫颈癌和宫颈炎患者的病原菌分布情况。结果 HPV DNA检测诊断宫颈癌的敏感性及准确性高于TCT、SCCA、CA15-3、CA125、CYFRA21-1检测及HPV E6/E7m RNA检测(P <0.05),且阳性预测值高。宫颈癌患者和宫颈炎患者生殖道感染均以革兰式阴性菌为主。结论 HPV DNA检测在宫颈癌诊断中具有较高的敏感性,是宫颈癌筛查的重要临床方法。宫颈癌组和宫颈炎组组生殖道感染均以革兰式阴性菌为主,其中宫颈癌合并生殖道感染患者阴道内以大肠埃希菌、肺炎克雷伯菌为主。
        Objective To explore the diagnostic value of HPV DNA infection and cervical cancer related examination indicators for cervical cancer, and to understand the distribution of pathogens of genital tract infection in patients with cervical cancer and chronic cervicitis. Methods50 patients with cervical cancer complicated with genital tract infection who came to our hospital from July 2015 to May 2017 were selected as observation group, and 50 patients with chronic cervicitis who came to our hospital during the same period were selected as control group. The sensitivity, accuracy, positive predictive value and negative predictive value of TCT, HPV DNA, SCCA, CA15-3, CA125, CYFRA21-1 and HPV E6/E7 in the diagnosis of cervical cancer were analyzed. The distribution of pathogenic bacteria in patients with cervical cancer and cervicitis was analyzed. Results The sensitivity and accuracy of HPV DNA detection in the diagnosis of cervical cancer were significantly higher than those of TCT, SCCA, CA15-3, CA125, CYFRA21-1 and HPV E6/E7(P < 0.05), and the positive predictive value was high. Genital tract infections in cervical cancer patients and cervicitis patients were mainly gram-negative bacteria. Conclusion HPV DNA detection has high sensitivity in the diagnosis of cervical cancer and is an important clinical method for cervical cancer screening. Genital tract infections in cervical cancer group and cervicitis group were mainly gram-negative bacteria. Escherichia coli and Klebsiella pneumoniae were predominant in vagina of patients with cervical cancer and genital tract infections.
