超声乳化联合前房角分离在白内障合并闭角型青光眼患者中的应用效果

摘要
目的研究超声乳化联合前房角分离在白内障合并闭角型青光眼患者中的应用效果。方法回顾性分析85例(92眼)白内障合并闭角型青光眼患者的临床资料,根据治疗方式分组,其中45例(49眼)采用超声乳化联合前房角分离术设为A组,40例(43眼)采用超声乳化联合小梁切除术设为B组。对比两组术前及术后1周的视力、眼压及前房深度及术后1周的并发症发生情况。结果术后1周,两组的视力及前房深度均增加、眼压均降低,且A组的视力及前房深度优于B组,眼压低于B组(P<0.05)。术后1周的并发症总发生率低于B组(P<0.05)。结论应用超声乳化联合前房角分离治疗白内障合并闭角型青光眼可明显降低眼压、改善患者视力和前房深度,且并发症发生率低,值得临床推广。
Objective To study the application effect of phacoemulsification combined with anterior goniosynechialysis in cataract patients with angle-closure glaucoma. Methods The clinical data of 85 cases(92 eyes) of cataract with angleclosure glaucoma were retrospectively analyzed. According to the treatment methods, 45 cases(49 eyes) treated with phacoemulsification combined with anterior goniosynechialysis were set as group A, while 40 cases(43 eyes) treated with phacoemulsification combined with trabeculectomy were set as group B. The visual acuity, intraocular pressure and anterior chamber depth before operation and at 1 week after operation and the incidence of complications at 1 week after operation was compared between the two groups. Results The visual acuity and anterior chamber depth increased and intraocular pressure decreased in both groups at 1 week after operation, and the visual acuity and anterior chamber depth in the group A were higher than those in the group B, the intraocular pressure was lower than that in the group B( P < 0.05). The total incidence of complications at 1 week after operation in the group A was lower than that in the group B(P<0.05). Conclusion Phacoemulsification combined with anterior goniosynechialysis in cataract patients with angle-closure glaucoma can significantly reduce intraocular pressure, improve visual acuity and anterior chamber depth, with low incidence of complications.
