抗抑郁治疗对脑卒中患者康复的影响

摘要
目的探讨对脑卒中患者予以抗抑郁治疗的效果。方法选取2016年1月～2017年4月在我院神经康复科就诊的42例脑卒中患者,按治疗方法的不同分成对照组(21例)与观察组(21例)。对照组患者采用常规药物治疗和康复治疗,观察组患者在对照组的基础上使用抗抑郁治疗,观察两组患者的康复效果。结果经治疗后,观察组患者的抑郁改善总有效率为90.48%(19/21),高于对照组的61.90%(13/21),差异有统计学意义(P<0.05);治疗4、6周后,观察组患者的神经功能缺损(NIHSS)评分分别为(6.35±1.20)、(4.30±1.03)分,低于对照组的(9.21±2.31)、(8.62±1.06)分,差异有统计学意义(P<0.05)。治疗4、6周后,观察组患者的日常生活能力(MBI)评分分别为(80.36±2.68)、(89.64±2.67)分,高于对照组的(70.31±4.32)、(80.03±4.68)分,差异有统计学意义(P<0.05)。此外,治疗后观察组患者的睡眠指数(PSQI)评分为(7.32±1.28)分,低于对照组的(10.25±2.67)分,差异有统计学意义(P<0.05)。结论对脑卒中患者采用抗抑郁治疗,可有效地改善患者抑郁情绪及神经缺损功能,还可提高患者的日常生活能力、改善睡眠质量,值得临床推广应用。
Objective To explore the influence of antidepressant treatment on rehabilitation of stroke patients. Methods A total of 42 stroke patients who were treated in our department of neurorehabilitation from January 2016 to April 2017 were enrolled. According to the different treatment methods, they were divided into the control group and the observation group, 21 cases in each group. In the control group, regular medication plus rehabilitation was used, while in the observation group, on the basis of the control group, antidepressant therapy was added. Rehabilitation effect of the two groups was observed. Results After treatment, the total effectiveness rate of depression improvement was 90.48%(19/21) in the observation group, which was higher than 61.90%(13/21) in the control group, with significant difference(P <0.05). After 4 and 6 weeks of treatments, the neurological deficit(NIHSS) scores of the observation group were(6.35±1.20) points and(4.30±1.03) points, respectively, which were lower than(9.21±2.31) points and(8.62±1.06)points of the control group, with statistical significance(P<0.05). After 4 and 6 weeks of treatments, the daily living ability(MBI) scores of the observation group were(80.36±2.68) points and(89.64±2.67) points, which were higher than(70.31±4.32) points and(80.03±4.68) points of the control group, with statistical significance(P<0.05). In addition, the sleep index(PSQI) score of the observation group was(7.32±1.28) points, which was lower than(10.25±2.67) points of the control group, with significant difference(P<0.05). Conclusion Antidepressant treatment for stroke patients can effectively improve the depression and neurological deficits of patients, enhance the daily living ability of patients and the quality of sleep, which is worthy of clinical promotion and application.
