左心耳容积对急性缺血性脑卒中后阵发性房颤的预测价值

摘要
目的探讨左心耳容积对急性缺血性脑卒中后并发阵发性房颤的预测价值。方法 115例急性缺血性脑卒中患者根据既往史及卒中后24 h长程动态心电图结果分为阵发性房颤组(45例)和非阵发性房颤组(70例),测量两组左心耳最大容积,采用多因素二元Logistics回归分析法分析急性缺血性脑卒中后阵发性房颤发生的危险因素,ROC曲线分析左心耳容积对卒中后阵发性房颤的预测价值。结果阵发性房颤组、非阵发性房颤组左心耳容积分别为(7.0±2.7)、(4.2±1.3) mL,两组比较,P <0.05。多因素二元Logistics回归分析显示,左心耳容积为急性缺血性脑卒中后并发阵发性房颤的独立危险因素(P <0.05)。左心耳容积> 5.25 mL时对急性缺血性脑卒中后阵发性房颤的预测价值最高(曲线下面积为0.825),灵敏度和特异度分别为66.7%和85.7%。结论左心耳容积是急性缺血性脑卒中后并发阵发性房颤的独立危险因素,其可以预测卒中后阵发性房颤的发生。
