艾滋病免疫重建炎性综合征并发麻风1例并文献复习

摘要
患者女,33岁。HIV-1抗体阳性7年。右上肢红斑和左上腹斑块4年。皮损组织病理示:表皮萎缩,部分侵蚀,真皮层见致密淋巴细胞包绕的上皮样细胞肉芽肿。皮肤组织液涂片抗酸染色查见抗酸阳性杆菌。诊断:艾滋病,界线类偏结核样型麻风。高效抗逆转录病毒治疗有反应,标准麻风联合化疗有效。
A woman 33-year-old with HIV-1 positive for 7 years was noted to have erythema in the right upper limb and plaque in the left upper abdomen for 4 years. The histopathology revealed epidermal atrophy with partial erosion. Epithelioid cells granuloma enveloped by dense lymphocytes was seen in the dermis. Skin tissue fluid for smears demonstrated positive acid fast stain. The diagnosis is AIDS with borderline tuberculoid leprosy( BT). The treatment of highly active antiretroviral treatment( HAART) and the standard multidrug therapy(MDT) is effective.
