经椎间孔镜髓核摘除与椎板开窗髓核摘除术治疗腰椎间盘突出症的比较分析

摘要
目的:比较经椎间孔镜髓核摘除(PELD)与椎板开窗髓核摘除术(FD)治疗腰椎间盘突出症(LDH)的临床疗效。方法:将112例LDH患者作为研究对象,应用随机数表法分为两组,每组各56例。对照组采取FD治疗,观察组行PELD治疗。观察两组临床疗效、围术期指标等。结果:与对照组比较,观察组手术时间较长,术中出血量较少,下地时间、住院时间较短,差异有统计学意义(P<0.05);手术前后两组间日本骨科协会评分(JOA)比较差异无统计学意义。结论:PELD是LDH治疗中一种较为有效且安全的术式,具有损伤小、术后恢复快等优势,利于缩短患者治疗时间,促进LDH转归。
Objective: To compare the clinical efficacy of transforaminal nucleus pulposus removal(PELD)and laminectomy nucleus pulposus(FD)in the treatment of lumbar disc herniation(LDH). Methods:112 patients with LDH were enrolled in the study. They were divided into two groups by random number table method, 56 cases in each group. The control group was treated with FD, and the observation group was treated with PELD. The clinical efficacy and perioperative index of the two groups were observed. Results:Compared with the control group, the operation time of the observation group was relatively long, the intraoperative blood loss was less, the time of the ground, and the hospitalization time were relatively short, and the difference was statistically significant(P<0.05). There was no significant difference in the scores for the Japanese orthopedic association scores(JOA) before and after the operation between the two groups. Conclusion: PELD is a more effective and safe procedure in the treatment of LDH. It has the advantages of small injury and quick recovery after operation. It is beneficial to shorten the treatment time of patients and accelerate the return of LDH.
