神经内镜下经蝶手术治疗巨大侵袭性垂体腺瘤的临床效果观察

摘要
目的:观察在巨大侵袭性垂体腺瘤的治疗中采取神经内镜下经蝶手术治疗的临床效果。方法:将笔者所在医院于2015年1月-2017年12月收治的以巨大侵袭性垂体腺瘤为诊断的患者112例,按随机数字表法分成对照组(56例)与试验组(56例)。试验组采取神经内镜下经蝶手术治疗,对照组采取显微镜下经蝶手术治疗,并对两组患者进行为期1年的随访。对比两组手术时长、术中出血量、住院总时长、肿瘤全切率、术后并发症发生率、复发率。结果:试验组的术中出血量、住院总时长均明显优于对照组,差异均有统计学意义(P<0.01)。试验组的肿瘤全切率为98.21%,术后并发症发生率为7.15%,复发率为1.79%,均明显优于对照组的87.50%、19.64%、12.50%,差异均有统计学意义(P<0.05)。结论:在巨大侵袭性垂体腺瘤的临床治疗中,采取神经内镜下经蝶手术治疗,术中创伤小,肿瘤全切率高,术后并发症与复发率更低。
        Objective:To observe the clinical effect of neuroendoscopic transsphenoidal surgery in the treatment of giant invasive pituitary adenomas.Method:A total of 112 patients with giant invasive pituitary adenomas were treated in author's hospital from January 2015 to December 2017.They were randomly divided into the control group(56 cases) and the experimental group(56 cases).The experimental group was treated with neuroendoscopic transsphenoidal surgery,while the control group was treated with transsphenoidal surgery under microscope.The patients in the two groups were followed up for one year.The two groups were compared in terms of operation time,intraoperative blood loss,total length of hospitalization,total tumor resection rate,incidence of postoperative complications and recurrence rate.Result:The intraoperative blood loss and the total length of hospitalization in the experimental group were significantly better than those of the control group,and the differences were statistically significant(P<0.01).The total tumor resection rate was 98.21%,the incidence of postoperative complications was 7.15%,the recurrence rate was 1.79% in the experimental group,were significantly better than the control group of 87.50%,19.64%,12.50%,and the differences were statistically significant(P<0.05).Conclusion:In the clinical treatment of giant invasive pituitary adenomas,neuroendoscopic transsphenoidal surgery has the advantages of less trauma,higher total tumor resection rate and lower incidence of postoperative complications and recurrence rate.
