荷芪散治疗肥胖型多囊卵巢综合征的临床观察

摘要
目的观察荷芪散联合二甲双胍治疗肥胖型多囊卵巢综合征患者的临床疗效。方法将60例肥胖型多囊卵巢综合征患者按随机表法分为对照组和治疗组,每组30例。对照组患者采用二甲双胍治疗,治疗组患者在对照组治疗基础上加荷芪散治疗,2组均治疗3个月。观察2组患者治疗后体质量指数(BMI)、空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDLC)、高密度脂蛋白胆固醇(HDL-C)、黄体生成素(LH)、促卵泡激素(FSH)、睾酮(T)、LH/FSH比值及卵巢大小变化情况,统计2组患者疗效及不良反应发生情况。结果 2组患者治疗后BMI、FPG、FINS、HOMA-IR、TC、TG、LDL-C、LH、LH/FSH、T及卵巢体积均显著降低(P均<0. 05),HDL-C、FSH均显著升高(P均<0. 05),且治疗组患者上述指标改善情况均显著优于对照组(P均<0. 05);对照组总有效率为63. 3%,治疗组总有效率为83. 3%,治疗组总有效率显著高于对照组(P <0. 05)。结论荷芪散联合二甲双胍治疗肥胖型多囊卵巢综合征不仅能明显降低患者的体质量,改善胰岛素抵抗,纠正糖脂代谢紊乱,而且还能改善患者性激素水平和卵巢体积,值得临床推广运用。
Objective It is to observe the clinical efficacy of Heqi Powder combined with metformin in the treatment of obese patients with polycystic ovary syndrome. Methods Sixty patients with obese polycystic ovary syndrome were randomly divided into control group and treatment group,30 cases in each group. The patients in the control group were treated with metformin. The patients in the treatment group were treated with Heqi Powder on the basis of the control group,both groups were treated for 3 months. After treatment,the changes of body mass index(BMI),fasting blood glucose(FPG),fasting insulin(FINS),insulin resistance index(HOMA-IR),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),luteinizing hormone(LH),follicle stimulating hormone(FSH),testosterone(T),LH/FSH ratio and ovarian size were observed before and after treatment in the two groups,the efficacy and adverse reactions occurred were assessed or recorded. Results The BMI,FPG,FINS,HOMA-IR,TC,TG,LDL-C,LH,LH/FSH,T and ovarian volume were significantly decreased,HDL-C and FSH were significantly increased in both groups after treatment(P < 0. 05),and the improvement of the above indicators in the treatment group was significantly better than that in the control group(P < 0. 05); the total effective rate in the treatment group was83. 3%,which was significantly higher than that of the control group(63. 3%)(P < 0. 05). Conclusion Heqi Powder combined with metformincan not only significantly reduce the body weight,improve insulin resistance,correct the disorder of glucose and lipid metabolism,but also improve the sex hormone level and ovarian volume of obese patients with polycystic ovary syndrome,it is worthy of clinical application.
