口腔护理预防ICU危重患者口腔感染及肺炎的效果

摘要
目的探讨不同口腔护理预防ICU危重患者口腔感染及医院获得性肺炎的效果。方法将我院ICU收治的79例危重患者随机分为观察组和对照组,对照组给予常规口腔护理措施,观察组给予综合性口腔护理干预,观察2组患者口腔清洁程度、口腔感染及医院获得性肺炎发生率。结果观察组口腔异味、污垢残留发生率及细菌培养阳性率显著低于对照组(P<0.05)。观察组口腔感染、肺炎及不良反应发生率均显著低于对照组,2组差异有统计学意义(P<0.05)。2组ICU住院初期口腔p H值差异无统计学意义(P>0.05),但观察组住院14 d及出院前口腔p H值均显著高于对照组,2组差异有统计学意义(P<0.05)。结论综合性口腔护理能显著提高口腔清洁程度,改善ICU危重患者口腔p H值,对于预防口腔感染及医院获得性肺炎有重要应用价值。
Objective To explore the effects of oral caring on prevention of oral infections and pneumonia in ICU patients. Methods According to a stratified randomization method,79 cases of critically ill ICU patients were divided into an observation group and a control group,and the control group received the routine oral care measures and the observation group was given of the comprehensive oral care measures. The cleaning degree,the incidence of oral infections and pneumonia were observed in the two groups. Results The bad breath,dirt residues and positive rate of bacterial culture in the observation group was significantly lower than those in the control group( P < 0. 05). The incidence rate of oral infections,pneumonia and adverse events in the observation group were significantly lower in the observation group than those in the control group,and there was a significant difference between two groups( P < 0. 05). There was no significant difference between the two groups in the patient's oral p H values in early days( P > 0. 05),but in the late hospitalization,the patient's oral p H values in the observation group was significantly higher than that in the control group,and there was a significant difference between two groups( P < 0. 05). Conclusions The comprehensive oral care measures can significantly improve the oral cleaning degree and p H value,and it has an important clinical value in the prevention of oral infections and pneumonia.
