正常高值血压人群无创血管功能检测的意义探讨

摘要
目的探讨在正常高值血压人群中应用无创血管功能检测的意义。方法在2013年1月至2018年1月医院体检的人群中分别选取100例血压正常者和100例正常高值血压者,另选取同期来院治疗的原发性高血压患者100例,分别将其记为A、B、C组。观察3组甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、C反应蛋白(CRP)、空腹血糖(GLU);使用动脉硬化检测仪检测3组的踝壁指数(ABI)、壁-踝脉搏波传导速度(baPWV),并对比3组的ABI、baPWV指标异常率。结果 3组TG、TC、LDL-C、CRP、GLU水平比较,差异具有统计学意义(P<0.05);C组TG、TC、LDL-C、CRP、GLU水平明显高于A组和B组,B组TG、TC、LDL-C、CRP、GLU水平高于A组,差异有统计学意义(P<0.05);3组间的ABI、baPWV值比较差异具有统计学意义(P<0.05),C组的ABI值均低于A组和B组(P<0.05),B组低于A组(P<0.05),C组的baPWV值高于A组、B组(P<0.05),B组高于A组(P<0.05);C组的ABI、baPWV指标异常率高于B组与A组(P<0.05),B组的异常率高于A组(P<0.05)。结论无创血管功能检测可反应正常高值血压人群动脉血管壁的病变情况,可为血压干预的治疗提供指导,从而预防血管的进一步病变。
Objective To explore the significance of noninvasive vascular function testing in patients with high-normal blood pressure.Methods 100 patients with normal blood pressure and 100 patients with high-normal blood pressure were selected from those who had been taking physical examinations in the Hospital between January 2013 and January 2018; another 100 patients with primary hypertension who were treated in the Hospital during the same period were selected; these patients were assigned into group A, B, and C, respectively. Three groups of triglycerides(TG), total cholesterol(TC), high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C), C-reactive protein(CRP) and fasting blood glucose(GLU) were observed.The Ankle Brachial Index(ABI) and brachial-ankle pulse wave velocity(baPWV) in the three groups were measured by arteriosclerosis tester, and the abnormal rates of ABI and baPWV indicators in the three groups were compared. Results The levels of TG, TC, LDL-C, CRP and GLU in the three groups were significantly different(P<0.05). The levels of TG, TC,LDL-C, CRP and Glu in group c were significantly higher than those in group a and group b, while the levels of TG, TC, LDL-C, CRP and Glu in group b were significantly higher than those in group a(P<0.05). There were significant differences in ABI and baPWV among the three groups. The ABI values in group C were lower than those in group A and group B(P<0.05), and those in group B were lower than group A(P<0.05). The baPWV value in group C was higher than that in group A and group B(P<0.05), and that in group B was higher than that in group A(P<0.05). The abnormal rate of ABI and baPWV in group C was higher than that in group B and group A(P<0.05), and the abnormal rate in group B was higher than that in group A(P<0.05). Conclusion Non-invasive vascular function test can reflect the ankle brachial lesions in patients with high-normal blood pressure, which can provide guidance for the treatment of blood pressure, thus preventing further pathological changes of blood vessels.
