功能锻炼联合心理引导干预对慢性阻塞性肺疾病稳定期患者肺功能、负性情绪及生存质量的影响

摘要
目的:探讨功能锻炼联合心理引导干预对慢性阻塞性肺疾病(COPD)稳定期患者肺功能、负性情绪及生存质量的影响。方法:通过对我院2015年4月-2017年5月就诊治疗的92例COPD稳定期患者临床资料做回顾性分析,并根据干预方式的不同分为观察组、对照组(各46例)。比较并分析两组患者干预前后FEV1、FVC、6min步行距离、SAS、SDS评分及生存质量评分。结果:干预后观察组FEV1、FVC、6min步行距离均较对照组上升明显(t=2.769,5.953,7.896;P<0.01),表明观察组患者肺功能改善更好。干预后观察组SAS、SDS评分均较对照组下降明显(t=-9.396,-9.556;P<0.001),表明观察组患者心理状况改善更好。观察组生理、心理、环境、社会关系各维度评分及生存质量总评分均较对照组上升明显(t=4.389,6.861,5.624,3.744,5.568;P<0.001)。结论:在COPD稳定期患者中应用功能锻炼联合心理引导干预可有效的改善患者负性情绪,促进肺功能恢复及提高生存质量,可作为临床推荐。
Objective:To explore the effect of functional exercise combined with psychological intervention on lung function,negative emotion and quality of life in patients with stable COPD.Methods:Analyze retrospectively,the clinical data of 92 patients with stable COPD treated in our hospital from April 2015 to May 2017,and according to different intervention methods,divided into observation group and control group(46 cases each).compare and analyze the FEV1,FVC,6 min walking distance,SAS,SDS scores and quality of life scores between the two groups before and after intervention.Results:After the intervention,the walking distance of FEV1,FVC and 6 min in the observation group was significantly higher than that in the control group(t=2.769,5.953,7.896;P<0.01),the lung function of the observation group improved better.After the intervention,the scores of SAS and SDS in the observation group were significantly lower than those in the control group(t=-9.396,-9.556;P<0.001),that the psychological condition of the observation group is better.Compare with the control group the dimensions of physiological,psychological and environmental groups,social relationship score and the total score of quality of life were significantly increased(t=4.389,6.861,5.624,3.744,5.568;P<0.001).Conclusion:Dirigation combined with psychological intervention can effectively improve negative emotions in patients with stable COPD,promote the recovery of lung function and improve the quality of life,can be recommended as a clinical.
