视频脑电图联合头颅MRI在儿童症状性癫痫定位诊断中的应用价值

摘要
目的:对视频脑电图(VEEG)联合头颅MRI在儿童症状性癫痫定位诊断中的应用价值进行分析。方法:选取我院于2017年1月至2018年12月收治的82例症状性癫痫患儿作为本次研究的对象,对VEEG发作期痫性放电(ID)、发作间痫性放电(IED)和头颅MRI的检查情况进行探讨,并且对VEEG和头颅MRI联合检查在症状性癫痫定位诊断中的价值进行分析。结果:(1)症状性癫痫临床发作情况:其发作类型以泛化、局灶性发作和痉挛发作为主。同时对病灶进行监测,可以看到ID为47例,IED为79例,MRI异常为62例,MRS异常为17例,其中MRS中有6例存在病理意义;(2)不同检查方式的检查结果比较:经过各组对比,可以看到ID联合ID联合头颅MRI定位的准确性均高于ID联合IED和IED联合头颅MRI定位的准确性,其差异显著(P<0.05);其余各组之间的对比均有一定差异性,但差异性并不显著(P>0.05)。结论:在针对症状性癫痫的定位诊断中,采用VEEG联合头颅MRI进行检查,其定位诊断准确性更高,具有较高的推广价值。
        Objective To analyze the value of video electroencephalogram(VEEG) combined with cranial MRI in the localization diagnosis of symptomatic epilepsy in children. Methods 82 children with symptomatic epilepsy admitted to our hospital from January 2017 to December 2018 were selected as the subjects of this study. The detection of epileptic discharge(ID), epileptic discharge(IED) and cranial MRI during VEEG seizures were discussed. The value of combined examination of VEEG and cranial MRI in the localization diagnosis of symptomatic epilepsy was analyzed. Results(1) The clinical seizures of symptomatic epilepsy were characterized by generalized seizures, focal seizures and spastic seizures. At the same time, monitoring the lesions, we can see that 47 cases had ID, 79 cases had IED, 62 cases had abnormal MRI and 17 cases had abnormal MRS, of which 6 cases had pathological significance;(2) Comparing the results of different examinations, we can see that the accuracy of ID combined with ID combined with cranial MRI is higher than that of ID combined with IED and IED combined with cranial MRI, and the difference is obvious.(P﹤0.05); there were some differences among the other groups, but the difference was not significant(P>0.05). Conclusion In the localization diagnosis of symptomatic epilepsy, VEEG combined with cranial magnetic resonance imaging is more accurate and has higher popularization value.
