补阳还五汤加减对胃肠道恶性肿瘤患者术后凝血功能及炎性因子的影响

摘要
目的:探讨补阳还五汤加减对胃肠道恶性肿瘤患者术后凝血功能及炎性因子的影响。方法:将接受手术治疗的胃肠道恶性肿瘤患者60例采用随机数字表法分为2组各30例。对照组采用常规方法治疗,观察组在常规治疗的基础上联合补阳还五汤加减治疗,2组均连续治疗15天。分别检测2组患者治疗前后凝血功能指标[纤维蛋白原(FIB)、凝血活酶时间(APTT)、凝血酶原时间(PT)、血小板计数(BPC)]和血清炎性因子[白细胞介素-2 (IL-2)、IL-6、肿瘤坏死因子-α(TNF-α)、前列腺素E2(PGE_2)]水平的变化情况。记录并统计2组治疗后不良反应发生率。结果:2组治疗前PT、APTT、FIB、BPC水平比较,差异无统计学意义(P> 0.05)。2组治疗后PT、APTT水平高于治疗前,FIB、BPC水平低于治疗前(P <0.05);观察组治疗后PT、APTT水平高于对照组,FIB、BPC水平低于对照组(P <0.05)。2组治疗前IL-2、IL-6、TNF-α、PGE2水平比较,差异无统计学意义(P> 0.05);2组治疗后IL-2、IL-6、TNF-α、PGE_2水平低于治疗前(P <0.05);观察组治疗后IL-2、IL-6、TNF-α、PGE_2水平低于对照组(P <0.05)。不良反应发生率观察组10.00%,对照组为13.33%,2组比较,差异无统计学意义(χ~2=1.583,P> 0.05)。结论:补阳还五汤用于胃肠道恶性肿瘤术后术后效果理想,有助于改善患者凝血功能,降低血清炎性因子水平,且药物安全性较高。
        Objective: To discuss the effect of modified Buyang Huanwu tang on the function of coagulation and fibrinolysis and levels of serum inflammatory factors of postoperative patients with gastrointestinal malignant tumor. Methods:60 cases of patients with gastrointestinal malignant tumor who received surgery for treatment were divided into two groups randomly,30 cases in each group. The control group received the routine treatment,while the observation group additionally received modified Buyang Huanwu tang. The treatment for both groups lasted for 15 days. Detected the changes of indexes of coagulation and fibrinolysis [fibrinogen(FIB), activated partial thromboplastin time(APTT),prothrombin time(PT),blood platelet count(BPC)] and levels of serum inflammatory factors [interleukin-2(IL-2), IL-6, tumor necrosis factor-α(TNF-α),prostaglandin E2(PGE_2)] of patients in both groups before and after treatment. Recorded and statistically analyzed the incidence of adverse reactions in both groups. Results: Before treatment, comparing the levels of PT, APTT, FIB and BPC in-between the two groups,there was no significant difference being found(P > 0.05). After treatment,the levels of PT and APTT in both groups were higher than those before treatment,and the levels of FIB and BPC were lower than those before treatment(P < 0.05);the levels of PT and APTT in the observation group were higher than those in the control group,and the levels of FIB and BPC were lower than those in the control group(P < 0.05). Before treatment,comparing the levels of IL-2,IL-6, TNF-α and PGE_2 in-between the two groups, there was no significant difference being found(P > 0.05). After treatment,the levels of IL-2,IL-6,TNF-α and PGE_2 in both groups were lower than those before treatment(P < 0.05);the levels of IL-2,IL-6,TNF-α and PGE_2 in the observation group were lower than those in the control group(P < 0.05).The incidence of adverse reactions was 10.00% in the observation group and 13.33% in the control group,difference being insignificant(χ~2= 1.583,P > 0.05). Conclusion:The application of modified Buyang Huanwu tang has optimal clinical effect in treating postoperative patients with gastrointestinal malignant tumor. It helps improve the function of coagulation function and reduce the levels of serum inflammatory factors with high medicine safety.
