摘要
目的探讨不同糖耐量状态下老年男性患者血清中FABP4水平变化及与颈动脉内膜中层厚度(CIMT)的相关性。方法选择新诊断T2DM患者52例,IGT患者40例、正常对照组(NGT)37例,采用ELISA方法检测血浆中FABP4水平,比较不同组间各脂代谢指标及CIMT的差异。结果血清FABP4在3组间呈现组间递增趋势,NGT(23.47±5.74)ng/m L、IGT(27.10±12.25)ng/m L、T2DM(33.68±15.14)ng/m L(P<0.05);CIMT在3组间亦呈递增趋势,NGT(0.93±0.28)mm、IGT(1.01±0.36)mm、T2DM(1.27±0.30)mm(P<0.05)。Pearson相关性分析显示,FABP4与SBP、BMI、WHR、TG、LDL-C、FPG、FINS、HOMA-IR、CIMT均呈正相关(P<0.01)。多元线性逐步回归分析显示,血清FABP4与CIMT、BMI、DBP、HOMA-IR独立相关(P<0.01)。结论老年男性人群血中FABP4与颈动脉硬化密切相关,加剧糖脂代谢紊乱发生。
        Objective To investigate the relationships between plasma adipocyte fatty acidbinding protein( FABP4) levels and carotid intima media thickness( CIMT) in elderly male patients with impaired glucose tolerance and type 2 diabetes. Methods A total of 129 elderly men were divided into three groups: normal glucose tolerance( NGT),impaired glucose tolerance( IGT) and type 2diabetes( T2DM). The levels of FABP4 were measured by enzyme-linked immunosorbent assay( ELISA). The associations between FABP4 and carotid intima media thickness were analyzed. Results Fasting FABP4 levels were higher in T2 DM group [( 33. 68 ± 15. 14) ng / m L ]and IGT[( 27.10 ± 12. 25) ng / m L]group than in NGT group[( 23. 47 ± 5. 74) ng / m L]( P < 0. 05). CIMT gradually increased among the three groups( P < 0. 05). There were significant positive correlation respectively between FABP4 and systolic pressure( SBP),body mass index( BMI),waist-hip ratio( WHR),triglyerides( TG),low density lipoprotein cholesterol( LDL-C),fasting plasma glucose( FPG),fasting insulin( FINS),insulin resistance index homeostasis model assessment( HOMA-IR)and CIMT( P < 0. 01). Multiple linear regression analysis indicated that CIMT,BMI,DBP and HOMA-IR were the independent factors affecting FABP4( all P < 0. 01). Conclusion FABP4 level may be associated with CIMT in elderly male patients,and aggravate the disorders of glucose and lipid metabolism.
