摘要
目的探讨体检人群代谢综合征(MS)组分与高同型半胱氨酸血症(HHcy)的相关性,为HHcy的防控提供科学依据。方法选择2017年1-10月新疆医科大学第一附属医院7 492例体检者为研究对象,进行问卷调查、体格检查及血生化检测。采用SPSS 22.0统计软件进行t检验及χ2检验。采用多因素逐步向前logistic回归分析HHcy患病的危险因素,采用单因素及多因素logistic回归分析MS组分数目与HHcy的相关性。结果在控制混杂因素后,多因素logistic回归分析结果显示,4项MS组分,即中心性肥胖(OR=1.35,95%CI:1.05~1.69)、高血糖(OR=1.24,95%CI:1.10~1.38)、高血压(OR=1.66,95%CI:1.46~1.83)和甘油三酯升高(OR=1.25,95%CI:1.02~1.52)是HHcy的危险因素(P<0.05,P<0.01)。1、2、3、4和5个MS组分的调查对象发生HHcy的风险分别是0个MS组分的调查对象的1.32倍(OR=1.32,95%CI:1.09~1.76)、1.94倍(OR=1.94,95%CI:1.63~2.99)、3.01倍(OR=3.01,95%CI:2.02~4.35)、4.47倍(OR=4.47,95%CI:2.76~5.71)和4.39倍(OR=4.39,95%CI:3.01~5.86)。结论中心性肥胖、高血糖、高血压、甘油三酯升高与新疆地区体检人群HHcy患病相关。MS人群应积极运动,养成良好的生活习惯和饮食习惯,降低血压、血糖、血脂水平,以控制同型半胱氨酸水平。
        Objective To explore the correlation between metabolic syndrome(MS) components and hyperhomocysteinemia(HHcy)in residents with physical examination of Xinjiang area,and to provide the scientific evidence for the prevention of HHcy.Methods From January to October in 2017,7 492 residents with physical examination in the First Affiliated Hospital of Xinjiang Medical University served as the subjects. The investigation was performed by questionnaire,physical examination and blood biochemical test. The multivariate stepwise forward logistic regression was used to analyze the risk factors of HHcy. Univariate and multivariate logistic regression were used to analyze the correlation between the number of MS components and HHcy. Results After controlling the confounding factors,multivariate logistic regression analysis showed that four abnormal MS components,i.e.central obesity(OR=1.35,95%CI:1.05-1.69),hyperglycemia(OR=1.24,95%CI:1.10-1.38),hypertension(OR=1.66,95%CI:1.46-1.83) and elevated triglyceride(OR =1.25,95%CI:1.02-1.52) were risk factors for HHcy,P<0.05,P<0.01. And the risk of HHcy in one,two,three,four and five abnormal MS components groups was 1.32(OR=1.32,95%CI:1.09-1.76),1.94(OR=1.94,95%CI:1.63-2.99),3.01(OR=3.01,95%CI:2.02-4.35),4.47(OR=4.47,95%CI:2.76-5.71) and 4.39(OR=4.39,95%CI:3.01-5.86) folds than the zero abnormal MS component group. Conclusion Central obesity,hyperglycemia,hypertension,elevated triglyceride were correlated with the occurrence of HHcy in the residents with physical examination of Xinjiang area.MS residents should actively exercise,develop good habits and dietary habits to reduce blood pressure,blood sugar,blood lipid levels for controlling the homocysteine level.
