乳腺自动容积成像汇聚征与常规超声边界不规整对乳腺癌的诊断价值

摘要
目的对比分析乳腺自动容积成像(ABVS)汇聚征与常规超声(US)边界不规整对≤2cm乳腺癌的诊断价值。方法对126例患者共140个≤2cm乳腺结节术前进行US及ABVS检查,对比分析US边界不规整与ABVS汇聚征诊断价值。结果 140个≤2cm乳腺结节,经术后病理证实,恶性72个,良性68个,US边界不规整诊断乳腺癌敏感度为80.6%、特异度为66.2%、准确度为73.6%、阳性预测值为71.6%、阴性预测值为76.3%;ABVS汇聚征诊断乳腺癌敏感度为61.1%、特异度为97.1%、准确度为78.6%、阳性预测值为95.7%、阴性预测值为70.2%,边界不规整诊断敏感度高于汇聚征,差异有统计学意义(χ~2=4.69,P=0.03<0.05),汇聚征诊断特异度高于边界不规整,差异有统计学意义(χ~2=19.05,P=0.00<0.05)。结论 ABVS汇聚征对诊断乳腺癌具有较高特异度,ABVS可作为US的补充方法用于乳腺疾病的诊断。
Objective To compare the value of convergent sign in automatic breast volume scanner(ABVS)system and uneven edge sign in ultrasound(US)when diagnosing breast nodules.Methods 126 patients with 140 breast nodules underwent preoperative examinations including ABVS and US.The diagnostic values of convergent sign in ABVS system and uneven edge in US were comparatively analyzed.Results Among the 140 breast nodules that were surgically removed,72 nodules were malignant and 68 nodules were benign.The sensitivity,specificity,accuracy positive,predictive and negative predictive rate of convergent sign in the diagnosis of breast malignant nodules were 80.6%,66.2%,73.6%,71.6% and 76.3%.The sensitivity,specificity,accuracy positive,predictive and negative predictive rate of uneven edge in the diagnosis of breast malignant nodules were 61.1%,97.1%,78.6%,95.7% and 70.2%.The sensitivity rate of convergent sign was higher than uneven edge,the difference was statistically significant(P<0.05).However,its specificity rate was lower than uneven edge.Conclusions The sensitivity rate of convergent sign in ABVS was high.ABVS could be used as a supplement to US in breast disease diagnose.
