甲状腺超声及甲状腺功能在老年患者甲状腺良恶性结节鉴别中的价值

摘要
目的探讨甲状腺超声及甲状腺功能在老年患者甲状腺良恶性结节鉴别中的价值。方法回顾性分析我院2017年6月至2018年6月期间诊断为甲状腺结节的60例患者的临床资料,根据诊断结果分为两组,观察组为良性结节患者(40例),对照组为恶性结节患者(20例),分析比较两组患者的甲状腺超声及甲状腺功能检查结果。结果观察组的超声影像显示回声情况、边界以及形态等特征与对照组存在统计学差异(P <0.05);两组的T3、 T4、 FT3以及FT4水平无统计学差异(P>0.05),观察组的TSH水平显著低于对照组(P <0.05)。结论在甲状腺良恶性结节的诊断中,甲状腺超声的诊断价值较高,可有效区分良性结节以及恶性结节; TSH水平的异常升高会促进结节的发生以及发展,可作为判断甲状腺良恶性结节的有效指标。
Objective To explore the value of thyroid ultrasound and thyroid function in the differential diagnosis of benign and malignant thyroid nodules in elderly patients. Methods The clinical data of 60 patients with benign and malignant thyroid nodules in our hospital from June 2017 to June 2018 were retrospectively analyzed. According to the diagnosis results, all cases were divided into observation group(benign nodules, n = 40) and control group(malignant nodules, n = 20). The results of thyroid ultrasound and thyroid function examinations were analyzed and compared between the two groups. Results The ultrasound images showed that there were significant differences in echo, boundary and morphology between the observation group and the control group(P <0.05). No significant difference was found between the two groups in the T3, T4, FT3 and FT4 levels(P >0.05), while the TSH level of the observation group was significantly lower than that of the control group(P <0.05). Conclusions In the diagnosis of benign and malignant thyroid nodules, thyroid ultrasound has high diagnostic value, which can effectively distinguish benign nodules from malignant nodules. The abnormal increase of TSH level will promote the occurrence and development of nodules, suggesting that TSH can be used as an effective indicator for judging the benign and malignant thyroid nodules.
