摘要
目的:重点分析剪切波超声弹性成像在乳腺良恶性肿瘤诊断中的应用价值。方法:对80例女性患者89个乳腺占位性病灶进行彩色多普勒超声和剪切波超声弹性成像检查,用彩色多普勒超声了解肿瘤的形状是否规则,有无包膜及占位感,纵横比,内部回声是否均匀,有无簇状钙化及微小钙化,后方回声衰减或增强,内部及周边血流有无,是否丰富及成角和穿支血流,再采用剪切波超声弹性成像技术,通过弹性图像和弹性最大值进行观察肿块的软硬度,以手术病理为金标准,分析剪切波超声弹性成像在诊断乳腺良恶性肿瘤中的准确度。结果:剪切波超声弹性成像在诊断乳腺良恶性病灶的准确度、敏感度、特异度分别为94.3%、94.5%、94.2%。结论:剪切波超声弹性成像在诊断乳腺良恶性病灶的准确度、敏感度、特异度都非常高,在乳腺占位性病变鉴别诊断中有着十分重要的作用。
        Objective: Selective analysis the value of shear wave ultrasound elastography in diagnosis of benign and malignant breast tumors. Methods: Color Doppler and shear wave ultrasound elastography examination of 89 breast space occupying lesions in 80 female patients, use color Dopplre to understand the form of tumor whether the shape of the tumor is regular, an envelope and a sense of occupying, longitudinal-trans-verse ratio(L/T), internal echo, small ang clustered calcifications, the rear echo attenuation or enhancement, inter and peripheral blood flow, internal and peripheral blood flow Whether or not there is abundance and angulation and blood flow. The soft and hardness of the masses were observed by using the elastic image and elastic maximum value of shear wave ultrasound elastography, surgical pathology as the gold standard, analysis of the accuracy of shear wave ultracsound elastography in the diagnosis of benign and malignant breast tumors. Results: The accuracy, sensitivity and specificity of shear wave ultrasound elastography in the diagnosis of benign and malignant breast lesions are 94.3%, 94.5%, 94.2%, Conclusion: The accuracy, sensitivity and specificity of shear wave ultrasound elastography are very high for benign and malignant breast lesions, there is a very important role in the diagnosis of breast space occupying lesions.
