鼻内镜下手术治疗复发性Ⅲ型慢性鼻-鼻窦炎鼻息肉的临床疗效评价

摘要
目的评价鼻内镜下手术治疗复发性Ⅲ型慢性鼻-鼻窦炎鼻息肉的临床疗效。方法 2015年8月—2017年8月,从耳鼻喉科收治的Ⅲ型慢性鼻-鼻窦炎鼻息肉患者中选取了120例,作为研究对象,将其以手术号单双数形式分成两组,常规手术组和鼻内镜手术组,各60例。两组用不同手术方式进行治疗。比较两组患者手术中出血量、并发症、治疗总有效率。结果常规组患者术中出血量是(46.34±9.56)ml,鼻内镜组患者术中出血量是(25.39±4.76)ml,两组比较差异有统计学意义(t=15.195,P<0.05);鼻内镜组患者中有11例出现了术后并发症,所占比例是18.33%,常规组患者中有25例出现了术后并发症,所占比例是41.67%,组间比较差异有统计学意义(χ~2=7.778,P<0.05);鼻内镜组患者总治愈率是81.67%,常规组患者总治愈率是55.00%,组间比较差异有统计学意义(χ~2=9.859,P<0.05)。结论鼻内镜手术治疗复发性Ⅲ型慢性鼻-鼻窦炎鼻息肉,减少术中出血量,术后并发症少,总有效率更高,临床疗效更好,值得临床推广。
Objective To evaluate the clinical efficacy of endoscopic sinus surgery for recurrent chronic rhinosinusitis and nasal polyps. Methods From August 2015 to August 2017, 120 patients with chronic rhinosinusitis and nasal polyps of type III were selected from the Department of otorhinolaryngology. They were divided into two groups in the form of singular and double numbers of operation numbers, 60 cases in each group. The routine operation group and the nasal endoscopic operation group were treated with different methods. The amount of bleeding, complications and the total effective rate of treatment were compared between the two groups. Results The intraoperative bleeding volume in the routine group was(46.34±9.56)ml, while that in the nasal endoscopy group was(25.39±4.76)ml, the t value was 15.195, P<0.05.There were significant differences between the two groups,with statistical significance; 11 patients in the nasal endoscopy group had postoperative complications, accounting for 18.33%,and 25 patients in the routine group had postoperative complications. The proportion was 41.67%. There were differences between groups(P<0.05). The χ~2 value was 7.778, with statistical difference. The total cure rate of nasal endoscopy group was 81.67%,and that of routine group was 55.00%. There were statistical differences between groups(χ~2=9.859, P<0.05). Conclusion Endoscopic sinus surgery for recurrent type III chronic rhinosinusitis and nasal polyps can reduce intraoperative bleeding and fewer complications. The total effective rate is higher and the clinical effect is better. It is worthy of clinical promotion.
