颅脑B超检查在新生儿缺血缺氧性脑病中的诊断价值

摘要
目的研究颅脑B超检查在新生儿缺血缺氧性脑病中的诊断价值。方法本次研究选取2016年5月～2018年3月期间诊治的49例新生儿缺血缺氧性脑病患儿(研究组);并选取同期49例正常足月新生儿(对照组);全部入选对象均在出生后24h内与出生后1～3d之间对其进行一次颅脑B超检查,观察分析两组新生儿的颅脑B超检查结果。结果全部入选对象的检查操作均完成后,观察分析组间的检查结果,对组间新生儿首次颅脑B超检查相关指标对比分析显示,研究组新生儿的大脑中动脉Vs(收缩期峰值血流速度)、Vm(时间平均血流速度)、RI(阻力指数)等指标水平明显高于对照组(P <0.05),而研究组大脑中动脉Vd(舒张末期血流速度)较对照组明显降低(P <0.05)。观察对比组间的第二次B超检查结果,研究组大脑中动脉Vs、Vm与Vd等指标水平明显高于对照组(P <0.05),而研究组的RI较对照组明显降低(P <0.05)。其中研究组患儿首次检查,有27例患儿检出异常(55.10%),而第二次检查,有45例患儿检出异常(91.83%)。结论新生儿缺血缺氧性脑病患儿应用颅脑B超检查诊断,具有显著的临床效果,可为患儿的临床诊治提供指导性依据。
Objective To study the diagnostic value of B-ultrasound examination in neonatal hypoxic ischemic encephalopathy. Methods Forty-nine cases of neonatal hypoxic-ischemic encephalopathy diagnosed and treated from May 2016 to March 2018 were selected as the study group. 49 normal full-term newborns in the same period were selected as control group. All the subjects were examined by B-mode ultrasonography within 24 hours after birth and between 1 and 3 days after birth. The results of B-mode ultrasonography were observed and analyzed. Results After the examination of all the selected subjects was completed, the results of the examination between the two groups were observed and analyzed. Comparative analysis of the first brain B-mode ultrasound examination showed that, The levels of Vs(peak systolic velocity), Vm(mean time velocity) and RI(resistance index) of middle cerebral artery in the study group were significantly higher than those in the control group(P < 0.05), while Vd(end diastolic velocity) of middle cerebral artery in the study group was significantly lower than that in the control group(P < 0.05). The results of the second B ultrasonic examination showed that the levels of Vs, Vm and Vd in the middle cerebral artery of the study group were significantly higher than those of the control group(P < 0.05), while the RI of the study group was significantly lower than that of the control group(P < 0.05). Among them, 27 cases(55.10%) were abnormal in the first examination, while 45 cases(91.83%) were abnormal in the second examination. Conclusion The application of B-mode ultrasonography in the diagnosis of neonatal hypoxic-ischemic encephalopathy has a significant clinical effect and can provide guidance for the clinical diagnosis and treatment of neonatal hypoxicischemic encephalopathy.
