摘要
目的探讨血浆鸢尾素水平与肌肉减少症的关系。方法 580例50岁以上健康体检者,非肌肉减少症343例为对照组,肌肉减少症患者237例为观察组,其中肌肉减少症前期123例(S1组),肌肉减少症中期71例(S2组),严重肌肉减少症期43例(S3组)。记录各组年龄、体质量指数、四肢骨骼肌质量、握力、步速等一般情况;采用ELISA法检测各组血浆鸢尾素水平并进行比较;采用Spearman相关分析鸢尾素水平与相对骨骼肌指数(relative appendicular skeletal muscle index, RASM)的相关性。结果观察组血浆鸢尾素水平[198.05(166.85,216.28)μg/L]低于对照组[252.91(230.90,274.08)μg/L](P<0.05);S1、S2、S3组血浆鸢尾素水平[211.10(198.42,226.80)μg/L、184.59(144.13,200.54)μg/L、162.00(142.33,186.25)μg/L]依次降低(P<0.05),均低于对照组(P<0.05);观察组血浆鸢尾素水平与RASM呈正相关(r=0.612,P<0.001)。结论随着肌肉减少症患者RASM的降低,其血浆鸢尾素水平逐渐减少;检测血浆鸢尾素水平有助于辅助诊断肌肉减少症。
        Objective To investigate the relationship between plasma irisin level and sarcopenia. Methods Totally 580 healthy volunteers aged 50 years or older included 343 cases of non-sarcopenia(control group) and 237 cases of sarcopenia(observation group). In observation group, there were 123 cases of pre-sarcopenia(group S1), 71 cases of mid-sarcopenia(group S2) and 43 cases of severe sarcopenia(group S3). The general data including age, body mass index, appendicular skeletal muscle mass, grip strength and walking speed were recorded in two groups. The levels of plasma irisin were detected by ELISA technique and compared among groups. Spearman correlation analysis was used to analyze the correlation between irisin levels and relative appendicular skeletal muscle index(RASM). Results The plasma irisin level was significantly lower in observation group(198.05(166.85, 216.28) μg/L) than that in control group(252.91(230.90, 274.08) μg/L)(P<0.05). The irisin level decreased gradually in group S1, S2 and S3 in turns(211.10(198.42, 226.80), 184.59(144.13, 200.54), 162.00(142.33, 186.25) μg/L)(P<0.05), all of which were significantly lower than that in control group(P<0.05). The plasma irisin level was positively correlated with RASM(r=0.612, P<0.001). Conclusion With the decrease of RASM in patients with sarcopenia, the level of plasma irisin decreases gradually. The detection of plasma irisin is helpful for the auxiliary diagnosis of sarcopenia.
