盐酸利托君联合硫酸镁治疗未足月早产胎膜早破的疗效及预后分析

摘要
目的 :探讨盐酸利托君联合硫酸镁治疗未足月早产胎膜早破疗效以及对患者预后影响。方法 :选取2015年3月～2016年2月在我院接受治疗的未足月早产胎膜早破患者92例,采用随机数表法分观察组和对照组,每组46例。对照组给予硫酸镁治疗,观察组给予盐酸利托君+硫酸镁治疗。记录患儿Apgar评分、出生体重,记录治疗前、治疗后白细胞介素-2(IL-2)、肿瘤坏死因子-α(TNF-α)、干扰素(IFN-y)、fFN(胎儿纤维连接蛋白)、IGFBP-1(胰岛素样生长因子结合蛋白)、MMP(线粒体膜电位)变化,记录患者恶心呕吐、胸闷、心率加快等不良反应发生率。结果 :治疗后,观察组新生儿Apgar评分、出生体重高于对照组;治疗后,患者IL-2、TNF-α以及IFN-y均下降,观察组低于对照组。治疗后患者的fFN、IGFBG-1以及MMP均低于治疗前,且观察组低于对照组;治疗后,患者均有出现恶心呕吐、胸闷、心率加快等不良反应,观察组发生率6.52%低于对照组23.91%。结论 :给予未足月早产胎膜早破患者盐酸利托君+硫酸镁治疗,可有效改善新生儿预后,降低不良反应发生率,降低患者炎症因子表达,改善fFN、IGFBP-1以及MMP分泌物,效果显著。
Objective To investigate the ritodrine hydrochloride combined with Magnesium Sulfate treatment of preterm premature rupture of membranes and the curative effects on the prognosis of the patients. Methods 92 cases of premature rupture of preterm premature ruapture of membranes were selected from March 2015 to February 2016 in our hospital, the patients were randomly divided into observation group and control group,. The control group was treated with Magnesium Sulfate, and the observation group was given ritodrine hydrochloride treatment + Magnesium Sulfate. Children with birth weight, Apgar score,interleukin-35 records before and after the treatment,-2(IL-2), tumor necrosis factor alpha(TNF-alpha) and interferon(IFN-y),fFN(fetal fibronectin) and IGFBP-1(insulin-like growth factor binding protein(MMP), mitochondrial membrane potential)changes were recorded. Vomiting, chest tightness, heart rate, incidence rate of adverse reactions was recorded. Results After treatment, the Apgar score and the birth weight of the observation group were higher than that of the control group; and after treatment, the observation group of neonatal Apgar score and birth weight was higher than the control group; and after treatment,the patients' IL-2, TNF-alpha and IFN-y were all decreased, and the observation group was lower than the control group. After treatment, the fFN, IGFBG-1 and MMP were lower than those before treatment, and the observation group was lower than that of the control group. and after treatment, the patients had adverse reactions such as nausea, vomiting, chest tightness and heart rate acceleration. The incidence of the observation group was 6.52% lower than that of the control group(23.91%). Conclusion For preterm premature rupture of membranes with ritodrine hydrochloride + Magnesium Sulfate treatment, can effectively improve the prognosis of newborns, reduce the incidence of adverse reactions, reduce the expression of inflammatory factors in patients with FN, IGFBP-1 and MMP, improve the secretion effect.
