清中汤联合PPI三联疗法治疗湿热型幽门螺杆菌胃溃疡的效果观察

摘要
目的:探讨清中汤联合PPI三联疗法治疗湿热型幽门螺杆菌(Hp)胃溃疡的效果。方法:选取2017年3月-2018年3月北京中医医院诊治的52例湿热型幽门螺杆菌胃溃疡患者,随机分为治疗组和对照组。治疗组以清中汤联合PPI三联疗法进行治疗,对照组以传统PPI三联疗法进行治疗,三联疗法依照2017版《消化性溃疡中西医结合诊疗共识意见》标准,疗程2周。采用《中药新药临床研究指导原则》中消化性溃疡症状分级量化标准比较两组治疗前后症状积分,幽门螺杆菌抑制率及胃镜检测溃疡情况。结果:治疗组治疗后症状总积分为(7.90±1.96)分,对照组治疗后症状总积分为(10.16±2.76)分,治疗组症状总积分明显低于对照组,且在纳差、大便异常、烦躁易怒症状上改善程度均优于对照组(P<0.05);治疗组Hp抑制率为92.31%,对照组Hp抑制率为73.08%,治疗组Hp抑制率高于对照组(P<0.05);胃镜检测溃疡示,治疗组总有效率为88.46%,对照组总有效率为65.38%,治疗组总有效率高于对照组(P<0.05)。结论:清中汤联合PPI三联疗法治疗湿热型幽门螺杆菌胃溃疡能提高临床效果,有效抑制Hp,优于传统PPI三联疗法,值得临床推广。
Objective:To investigate the therapeutic effect of Qingzhong Decoction combined with PPI triple therapy on Helicobacter pylori(Hp) gastric ulcer.Method:A total of 52 patients with damp-heat Helicobacter pylori gastric ulcer who were treated in Beijing Chinese Medicine Hospital from March 2017 to March 2018 were randomly divided into the treatment group and the control group.The treatment group was treated with Qingzhong Decoction combined with PPI triple therapy.The control group was treated with traditional PPI triple therapy.The triple therapy was performed according to the 2017 edition of the Consensus on the Diagnosis and Treatment of Peptic Ulcer with Integrated Traditional Chinese and Western Medicine,and treatment 2 weeks.According to the Guidelines for Clinical Research of New Drugs in Traditional Chinese Medicine of the quantitative criteria for the classification of peptic ulcer symptoms,the symptom scores were compared between the two groups before and after treatment,and inhibition rate of Helicobacter pylori and gastroscopic examination of ulcers.Result:After treatment,the total score of the symptoms in the treatment group was(7.90±1.96)points,and the total score of the symptoms in the control group was(10.16±2.76)points,the treatment group was significantly lower than the control group,and the improvement degree of anorexia,abnormal stool,and irritability was better than the control group(P<0.05).The Hp inhibition rate of the treatment group was 92.31%,and the Hp inhibition rate of the control group was 73.08%.The treatment group was higher than the control group(P<0.05).The results of gastroscopy showed ulcers,the effective rate of the ulcer of the treatment group was 88.46%,and the effective rate of the ulcer of the control group was 65.38%,and the treatment group was higher than the control group(P<0.05).Concluslon:Qingzhong Decoction combined with PPI triple therapy for the treatment of damp-heat Helicobacter pylori gastric ulcer can improve the clinical efficacy and effectively inhibit Hp,which is superior to traditional PPI triple therapy and worthy of clinical promotion.
