社区高血压应用综合防治健康管理模式的治疗效果

摘要
目的研究综合防治健康管理模式在社区高血压患者中的应用效果。方法选取2017年3月至2018年3月佛山市顺德区乐从社区卫生服务中心管理的接受常规药物治疗与综合防治健康管理的高血压患者200例作为观察组,同期接受常规药物治疗和健康教育的高血压患者200例作为对照组,通过1年的干预,比较两组患者的血压水平、生活行为和相关检测指标。结果通过1年的干预,观察组患者生活行为指标中的适当运动、低盐摄入、低糖摄入、低脂摄入、戒烟限酒、心理平衡、服药依从、遵医行为人数均明显多于对照组,差异均有统计学意义(P<0.05);观察组患者的收缩压(SBP)及舒张压(DBP)分别为(126.19±5.18) mmHg、(82.44±6.14) mmHg,明显优于对照组的(134.47±5.25) mmHg、(90.74±6.84) mmHg和自身干预前的(175.25±10.46) mmHg、(108.07±6.87) mmHg,差异均有统计学意义(P<0.05);观察组患者的血压、血糖、血脂、体重达标率和高血压知识、非药物治疗知晓率均明显高于对照组,差异均有统计学意义(P<0.05)。结论综合防治健康管理模式有利于促使社区高血压患者养成良好的生活行为习惯,维护并改善患者的健康状态,提高血压控制效果,值得推广应用。
Objective To study the effect of comprehensive health management model on hypertensive patients in community. Methods A total of 200 patients with hypertension who received routine drug treatment and comprehensive health management by Lecong Community Health Service Center of Shunde District of Foshan City from March 2017 to March 2018 were selected as the observation group, and 200 patients with hypertension who received routine drug treatment and health education were enrolled as the control group during the same period. All patients received intervention for a period of one year. The blood pressure level, living behavior, and relative test indexes of two groups of patients were observed and compared. Results The number of patients with healthy exercise, low salt intake, low sugar intake, low fat intake, smoking cessation, alcohol restriction, psychological balance, drug compliance, and docile behavior in the observation group were significantly higher than those in the control group. The systolic blood pressure and diastolic blood pressure in the observation group after treatment were(126.19±5.18) mmHg and(82.44±6.14) mmHg, respectively, which were significantly better than(134.47 ± 5.25) mmHg and(90.74 ± 6.84) mmmHg in the control group and(175.25±10.46) mmHg and(108.07±6.87) mmHg in the observation group before treatment(P<0.05). The awareness rate of the knowledge of blood pressure, blood sugar, blood lipid, body weight, hypertension, and non-drug treatment in the observation group were significantly higher than those in the control group, and the differences were statistically significant(P<0.05). Conclusion The comprehensive prevention health management mode can help the community hypertensive patients develop good living habits, maintain and improve the health status, and promote the blood pressure control effect, which is worth promoting and applying.
