多层螺旋CT在甲状腺癌临床诊断中的应用效果

摘要
目的探讨多层螺旋CT(MSCT)在甲状腺癌临床诊断中的应用效果。方法回顾性分析本院经病理检查证实为甲状腺癌的63例患者的临床资料,均接受彩色多普勒超声(CDUS)与MSCT检查,对比两组病灶检查情况。结果 63例患者中经病理检查证实有47例单发甲状腺癌,16例多发性甲状腺癌(包括12例2个癌灶,4例3个癌灶);以病理检查结果作对照,CDUS、MSCT的诊断符合率分别为62.65%、78.31%,MSCT的诊断符合率高于CDUS(P<0.05);MSCT对甲状腺癌边缘模糊的检出率较CDUS高(P<0.05),对甲状腺癌其他影像图特征的检出率与CDUS比较无明显差异(P>0.05);MSCT对颈部转移性淋巴结的检出率78.26%略较CDUS的52.17%高,但差异无统计学意义(P>0.05)。结论 MSCT在甲状腺癌临床诊断中的诊断效果好于CDUS,可在一定程度上提高诊断准确性,减少误漏诊的发生。
Objective To investigate application effect of multi-slice spiral CT(MSCT) in clinical diagnosis of thyroid cancer. Methods Reviwe and analyze clinical data of 63 cases diagnosed thyroid cancer patients with pathological examination in our hospital. Compare lesions examination of two groups after color Doppler ultrasound(CDUS) and MSCT test. Results Among 63 cases, 47 were diagnosed with pathological examination as sporadic thyroid cancer, 16 cases of multiple thyroid cancer(including 12 cases with 2 lesions and 4 cases with 3 lesions). Based on pathological examination, diagnostic accordance rate of CDUS and MSCT was 62.65% and 78.31% respectively. MSCT diagnostic accordance rate was higher than CDUS(P<0.05); detection rate of thyroid cancer blurred margin of MSCT was higher than CDUS(P<0.05). Detection rate of other imaging features of thyroid cancer showed no significant difference from CDUS(P>0.05); detection rate of cervical metastatic lymph nodes of MSCT was 78.26%, slightly higher than 52.17% of CDUS, but there was no significant difference,(P>0.05). Conclusion MSCT has better diagnosis effect than CDUS for thyroid cancer, which can improve diagnosis accuracy and reduce occurrence of misdiagnosis to some certain extent.
