摘要
目的:评价微柱凝胶技术用于检测孕妇血型抗体IgG亚型的可靠性以及IgG亚型与新生儿溶血病的相关性。方法:采用自主研发的IgG亚型微柱凝胶反应卡,对47例产前夫妇抗体效价高的孕妇测定其IgG亚类抗体的效价,其中ABO血型系统38例,Rh系统9例,对产后出现溶血病的患儿亦监测其IgG亚型抗体水平,35例抗体效价正常的孕妇及其产后新生儿作为对照组。结果:47例血型抗体高效价的孕妇血清中检测出26例IgG1和IgG3亚型同步升高,产后新生儿均出现了不同程度的高胆红素血症,21例IgG2和IgG4亚型升高,产后新生儿未出现高胆红素血症表现。结论:微柱凝胶技术用于检测孕妇血型抗体IgG亚型,方法简单快速、结果直观可靠,可以在普通实验室常规应用,对高效价孕妇有必要进行IgG亚型分析,当检出IgG1或IgG3亚型升高时给予预防性干预治疗,而如果仅IgG2或IgG4亚型升高,孕妇可以不进行早期干预治疗,从而实现精准化干预治疗。
Objective:To evaluate the reliability of the micro column gel technique assay(MGTA)in the detection of blood type antibodies IgG subsets and the relationships between neonatal hemolytic disease and the IgG subsets.Method:The independent innovative method of microcolum gel technology in IgG subset detection was established.The IgG subsets were analyzed on 47 cases pregnant women(ABO 38 cases and Rh 9 cases)who had the high titer antibodies and the neonatal sera of HDN.At the same time 35 cases healthy gravidaes and their infants were control group.Result:There were 26 pregnant women in totle 47 cases,and their concentration of IgG1 or IgG3 were higher than those in normal group(P<0.05),neonates were vulnerable to HDN.It was certificated the21 pregnant women with IgG2 or IgG4,and neonates were not vulnerable to HDN.Conclusion:The MGTA may have the advantages of easy procedure and high accuracy in screening the contents of IgG subsets to be propitious for general clinical application.The method may provide a directional significance in early intervention of blood group incompatibility hemolytic disease of the newborns.
