摘要
目的探讨吸烟与高尿酸血症(HUA)对觉醒卒中的交互作用。方法选择我院神经内科收治的觉醒卒中患者70例为觉醒卒中组,同时选取同期于本院体检健康者72例为对照组。比较并分析两组人群的年龄、性别、生活习惯以及血尿酸水平,并采用多因素回归的方法分析吸烟与高尿酸血症与觉醒卒中的相关性,以及利用相加模型观察二者对觉醒卒中发病的交互作用。结果觉醒卒中组吸烟率以及血尿酸水平高于对照组(P <0.01)。多因素Logistic回归分析显示,吸烟者觉醒卒中的风险是不吸烟者的3.423倍,高尿酸血症者患觉醒卒中的风险是血尿酸正常者的7.737倍。二者均匀明显的统计学意义(P均<0.01),年龄、性别、饮酒史不具有统计学意义(P>0.05)。Logistic回归相加模型分析显示,吸烟并高尿酸血症者患觉醒卒中的风险是不吸烟并血尿酸正常者的2.541倍,吸烟与高尿酸血症同时存在的交互作用指数是1.802,纯交互作用归因比为43.61%。结论吸烟及高尿酸血症是觉醒卒中的两个独立危险因素,对觉醒卒中的发病有正交互作用,应当给予积极的干预,从而降低觉醒卒中的发病率。
        Objective To investigate the interaction between smoking and hyperuricemia(H-UA)on arousal stroke.Methods From January 2014 to January 2018,70 patients with arousal stroke admitted to the Department of Neurology,Beijing Daxing District Hospital of Integrated Traditional Chinese and Western Medicine were selected as the arousal stroke group.72 patients who were healthy in the same period were selected as the control group.The age,sex,living habits,and serum uric acid levels of the two groups were compared and analyzed.Multivariate logistic regression was used to analyze the correlation between smoking/hyperuricemia and arousal stroke.And using the additive model to observe the interaction between the two factors on arousal stroke.Results The smoking rate and blood uric acid level in the arousal stroke group were higher than those in the control group,and the difference was statistically significant(P<0.01).Multivariate logistic regression analysis showed that the risk of awakening stroke in smokers was 3.423 times that of non-smokers,and the risk of arousal stroke in hyperuricemia was 7.737 times that of normal serum uric acid.The two were statistically significant(P<0.01),age,gender,and drinking history were not statistically significant(P>0.05).Logistic regression and additive model analysis showed that people with smoking and hyperuricemia were awakened.The risk of stroke was 2.451 times that of non-smokers and normal uric acid.The interaction index of smoking and hyperuricemia was 1.802,and the pure interaction attribution ratio was 43.61%.Conclusion Smoking and hyperuricemia are two independent risk factors for arousal stroke.They have positive interactions with the onset of arousal stroke.Active intervention should be given to reduce the incidence of arousal stroke.
