摘要
目的探讨硒与糖尿病相关代谢因素之间的关系。方法按照多阶段分层整群随机抽样的原则调查山西省人群1 402例,抽取空腹静脉血,测定血清硒(Se)、糖化血红蛋白(Hb A1c)、空腹血糖(FBG)、OGTT-2h血糖、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、血尿酸(UA)。分析不同血硒水平与葡萄糖及脂代谢之间的关系。结果 (1)调查的山西省1 402例人群中有1 049例血硒值低于正常(1.60~3.00μmol/L),占总人群的74.82%。(2)血硒值≤0.50μmol/L组,血硒水平与HbA_1c水平呈明显的负相关(r=-0.349,P<0.05),提示严重的低硒状态与血糖水平升高密切相关。(3)血硒值在0.51~1.00μmol/L与1.01~1.59μmol/L2组,血硒水平与HbA_1c、FBG、OGTT-2 h、TG、TC、LDL-C、HDL-C、UA之间未见明显相关性(P>0.05)。(4)血硒值在1.60~3.00μmol/L组,血硒水平与TG呈明显的正相关(r=0.189,P<0.01),提示血硒水平增高对甘油三酯代谢有正性影响。(5)血硒值>3.00μmol/L组,血硒水平与LDL-C呈明显的负相关(r=-0.176,P<0.05),提示富硒状态对低密度脂蛋白具有降低作用。结论山西省属低硒地区,且人群硒营养状况分布不均,血硒水平异常与糖代谢及脂质代谢紊乱密切相关,严重的低硒状态与血糖水平升高密切相关,富硒状态对低密度脂蛋白具有降低作用。
        Objective To investigate the relationship between selenium and diabetes related metabolic factors in different blood selenium levels. Methods According to the principle of multi-stage stratified cluster random sampling,We investigated the population of Shanxi province in 1 402 cases, fasting venous blood, testing the standards of serum selenium( Se),glycosylated hemoglobin( HbA_1c), fasting blood glucose( FBG), OGTT-2h blood glucose, triglyceride( TG), total cholesterol( TC), low density lipoprotein cholesterol( LDL-C), high density lipoprotein cholesterol( HDL-C) and the blood uric acid( UA), and analyzing the relationship between different blood selenium levels and metabolism of glucose and lipid.Results(1)Survey of 1402 people in Shanxi,there were 1049 cases of blood selenium level was lower than normal( 1.60 ~3μmol/L), accounting for about 74.82% of the total population. There was no significant difference between rural population and urban population( P>0.05).(2)The blood selenium value is less than or equal to 0.50 μmol/L group, blood selenium level was negatively correlated with the level of HbA_1c increased( r=-0.349, P<0.05); that serious low selenium status and glucose levels are closely related.(3)In the two group,the selenium content in blood was from 0.51 to 1 μmol/L and from 1.01 to 1.59μmol/L, there was no significant correlation between serum selenium level and HbA_1c, FBG, OGTT-2h, TG, TC, LDL-C,HDL-C, UA( P>0.05).(4)The blood selenium level in 1.60 ~ 3 μmol/L group, the blood selenium level and TG showed a significant positive correlation( r=0.189, P<0.01), suggesting that the increase in blood selenium level has a positive correlation with triglyceride metabolism.(5) In the group of blood selenium value > 3 mol/L,there is a significant negative correlation with the blood selenium level of LDL-C( r=-0.176, P<0.05), suggesting that selenium can reduce the effect on the state of low density lipoprotein. Conclusion Shanxi province belongs to low selenium area, and the distribution of selenium in the population is uneven. The abnormal level of blood selenium is closely related to glucose metabolism and lipid metabolism disorder. Severe low selenium status is associated with elevated blood glucose levels.Selenium rich state can reduce the effect of lowdensity lipoprotein.
