摘要
目的:采用256层螺旋计算机断层摄影术(MSCT)定量测量正常成人不同时相冠状静脉系统参数,提出不同时相冠状静脉窦口及其分支静脉开口径线、截面积的正常参考值范围。方法:选取临床可疑冠心病或无症状体检的受试者70例, MSCT冠状动脉成像结果并未发现冠状动脉病变,无冠状动脉畸形、变异,均为右冠状动脉优势型,对原始图像选取心脏收缩期(45%时相)、心脏舒张期(75%时相)进行重建,测量冠状静脉窦口及其分支静脉开口径线、截面积,并对心动周期不同时相各测量值进行比较。结果:(1)心动周期不同时相,冠状静脉窦口及其分支静脉开口径线、截面积45%时相数值均较75%时相增大(P<0.05)。(2)心动周期同一时相,冠状静脉窦口及其分支静脉开口径线、截面积男性与女性数值无差异(P>0.05);按照身高、体重计算截面积进行标化,在同一时相冠状静脉窦及其分支静脉开口径线、截面积男性与女性数值差异仍无统计学意义(P>0.05)。(3)提出45%及75%时相冠状静脉窦口及其分支静脉开口径线、截面积95%参考范围。结论:采用MSCT显示冠状静脉成像可以客观定量测量、分析不同时相冠状静脉窦口及其分支静脉开口径线、截面积,其正常参考值提出为临床成功实行心脏介入手术提供了重要影像参考价值。
        Objectives: To quantitatively measure the coronary system parameters at different phase of cardiac cycle in normal adults by 256-slice computed tomography, and to provide normal reference ranges of coronary sinus, mainly branch diameter,sectional area in different phase of cardiac cycle.Methods:A total of 70 patients(30 men, 40 women; mean age [49.89±10.85] years) were selected in this analysis. These patients were examined by MSCT with suspected coronary heart disease, and MSCT results showed no coronary artery disease(no coronary artery deformity, variation, right dominant type of coronary artery) and no other heart disease in these patients. Systole phase(45%) and diastolic phase image(75%) of the original image were reconstructed, and quantitatively measured coronary sinus, mainly branch diameter,sectional area, and to analyze the relationship between the above imaging indexes with the different phase of cardiac cycle. Results: 1. During the different phase of cardiac cycle, the values of coronary sinus, mainly branch diameter,sectional area are larger in 45% phase than those in 75% phase(P<0.05). 2.During the same phase of cardiac cycle, the coronary sinus, mainly branch diameter,sectional area were similar between males and females(P>0.05), these parameters were also similar between males and females after standardization by heights and weights(P>0.05). 3. During the 45% interval and 75% interval, there were significant difference in the 95% reference ranges of coronary sinus, mainly branch diameter,sectional area.Conclusions: 256-slice spiral CT can be used as an objective quantitative measurement of coronary vein main branch diameter, coronary vein sectional area in different phase of cardiac cycle, which provides an important image reference value for the successful implementation of cardiac intervention procedures.
