集束化护理干预在预防乳腺癌患者植入式中心静脉输液港并发症中的应用价值

摘要
目的研究集束化护理干预在预防乳腺癌患者植入式中心静脉输液港并发症中的应用价值。方法选取我院乳腺科2017年1月至2018年8月140例乳腺癌住院治疗并同期手术的患者,并按照随机数字表将患者随机分成试验组70人和对照组70人,对照组采用常规护理,试验组在常规护理的基础上采用集束化护理干预措施。结果试验组在实施集束化干预措施后出现非计划性拔管1例,静脉血栓1例,回抽无血2例,继发感染3例,导管移位2例,气胸1例,血胸1例,无导管阻塞、药液外渗;对照组非计划性拔管4例,静脉血栓5例,回抽无血7例,继发感染9例,导管移位6例,气胸3例,血胸2例,导管阻塞4例,药液外渗5例;两组比较差异有统计学意义(P<0.05)。集束化护理干预后,观察组患者在生理维度、社会/家庭维度、情感维度、功能维度、乳腺癌特异维度等5个维度和FACT-B量表总生命质量方面评分均高于对照组;而在社会/家庭维度、功能维度、FACT-B量表总生命质量方面,两组得分比较差异均具有统计学意义(P<0.05)。结论集束化护理干预能很好的降低乳腺癌患者植入式中心静脉输液港并发症的发生,同时提高了患者的生活质量。
        Objective To study the application value of Ventilator Care Bundles in preventing complications of implanted central venous infusion port in breast cancer patients. Methods The patients of 140 cases of breast cancer in our hospital from January 2017 to August 2018 were selected, and 70 patients were randomLy divided into 70 in the trial group and 70 in the control group according to the random digital table. The experimental group adopted Ventilator Care Bundles on the basis of routine nursing. Results 1 case of unplanned tubing, 1 case of venous thrombosis, 2 cases of hemoptysis, 3 cases of secondary infection, 2 cases of catheter transposition, 1 case of pneumothorax, 1 case of hemothorax, 1 case of non-catheter obstruction and exosmosis; In the control group, there were 4 cases of unplanned drainage, 5 cases of venous thrombosis, 7 cases of no blood recovery, 9 cases of secondary infection, 6 cases of catheter displacement, 3 cases of pneumothorax, 2 cases of blood chest, 4 cases of catheter obstruction, and 5 cases of drug extravasation; The difference between the two groups was statistically significant(P<0.05). After Ventilator Care Bundles, the patients in the observation group scored higher than the control group in five dimensions such as physiological dimension, social/family dimension, emotional dimension, function dimension, breast cancer specific dimension, and the total quality of life of the FACT-B scale. In terms of the social/family dimension, functional dimension, and the total quality of life of the FACT-B scale, the two groups scored statistically significant differences(P<0.05). Conclusion Ventilator Care Bundles can reduce the complication of implantable central venous infusion port and improve the quality of life of breast cancer patients.
