摘要
目的了解宁夏男男性行为(MSM)人群HIV-1的流行亚型分布及耐药基因变异情况。方法采用方便抽样方法,抽取宁夏籍男男性行为人群HIV-1感染者共33份血浆标本,进行反转录/巢式PCR扩增HIV病毒的pol基因片段、DNA测序、亚型鉴定,并与国际HIV耐药数据库进行比对分析。结果宁夏MSM人群HIV-1毒株总体上未见明显的区域聚集性,33例样本呈现4种基因亚型分布:CRF07_BC、CRF01_AE、B、CRF55_01B,优势毒株为CRF07_BC(66.67%)和CRF01_AE(18.18%)亚型。CRF07_BC亚型序列主要形成2个基因簇。发现7例耐药突变株,全部为原发耐药,5例产生NRTIs与NNRTIs交叉耐药。结论宁夏MSM人群HIV-1流行毒株亚型分布较复杂,重组型毒株已成为优势流行株。原发耐药发生率高,交叉耐药明显,暂未见传播耐药株。
        Objective To investigate the subtype distribution and gene mutations and drug resistance of human immunodeficiency virus(HIV)-1among men who have sex with men(MSM) in Ningxia.Methods A convenient sampling method was adopted to collect 33 plasma specimens from HIV-1 infected persons among MSM in Ningxia.And RNA was extracted and reverse transcription PCR and nested PCR for amplification of pol gene fragments,and DNA was sequenced and then phylogenetically analyzed.Finally subtypes were identified and drug resistance was analyzed in comparison with International HIV drug resistance database.Results In general,there was no obvious regional aggregation in the MSM HIV-1 strain in Ningxia.The samples presented in 33 samples showed 4 genotype subtypes:CRF07 _ BC,CRF01 _ AE,B,CRF55 _ 01 B,and the dominant strains were CRF07 _ BC(66.67%) and CRF01 _ AE(18.18%) subtypes.The CRF07_BC subtype sequence mainly formed 2 gene clusters.Seven drug resistant mutant strains were found,all of which were resistant to the drug,and 5 were the drug resistance of NRTIs and NNRTIs.Conclusion In the MSM population of Ningxia,the subtype distribution of HIV-1 popular strains is more complex,and the regroup strain has become an advantage.The incidence of drug resistance is high and the cross resistance is obvious,but no resistant strain is found.
