腹腔镜手术治疗卵巢子宫内膜异位囊肿临床研究

摘要
目的探讨腹腔镜与开腹手术治疗卵巢子宫内膜异位囊肿的临床效果。方法对2010年5月~2013年4月就诊于本院的190例卵巢子宫内膜异位囊肿患者的临床资料进行回顾性分析,按照所采取的治疗方法分为腹腔镜组及开腹组(n=95)。两组患者的各项指标采用统计学方法比较。结果腹腔镜组与开腹组比较,术中出血量明显较少,术后住院时间及应用抗生素时间均明显较短,术后病率、术后镇痛率均明显较低,两组间具有显著性差异(P<0.05)。两组手术时间、切口甲级愈合率均无显著性差异(P>0.05)。腹腔镜组患者FSH水平在术后首次月经第3天与术前及对照组比较明显较高,而E2水平首次月经第3天与术前及对照组比较明显较高,均有显著性差异(P<0.05)。结论卵巢子宫内膜异位囊肿患者采用腹腔镜进行手术治疗能够收到较好的效果,远期安全性较高,但需对适应证进行严格的掌握。
Objective To investigate the clinical effect of ovarian endometriosis cyst patients treated with laparoscopic and open operation.Methods The clinical data of 190 cases of ovarian endometriosis cyst were collected from 2010May to 2013 April and were retrospectively analyzed,they were divided into laparoscopic group and laparotomy group according to the treatment method(n=95).All the indexes of two groups were compared with statistical method.Results Bleeding of the laparoscopic group was significantly less than the laparotomy group,postoperative hospitalization time and antibiotics time were significantly shorter,postoperative morbidity,postoperative analgesia were significantly lower,there were significant differences between two groups(P<0.05).The operation time,incision healing rates of the two groups were not significantly differences(P>0.05).FSH levels in patients with laparoscopic group on postoperative day 3 for the first time menstruation compared with before operation and the control group obviously higher,and E2level for the first time period with 3 days before operation and the control group was significantly higher,had significant difference(P<0.05).Conclusion Ovarian endometriosis cyst were treated by laparoscopic operation treatment can receive better effect.The forward security is higher,but to be carried out in strict control of indications.
