小剂量枸橼酸咖啡因联合氨茶碱治疗早产低体重呼吸暂停患儿的效果分析

摘要
目的 :探讨小剂量枸橼酸咖啡因联合氨茶碱治疗早产低体重呼吸暂停患儿的效果。方法 :选取2015年1月~2016年1月收治的92例早产低体重呼吸暂停患儿采用前瞻性随机研究,分为联合组、对照组各46例,联合组采用小剂量枸橼酸咖啡因联合氨茶碱治疗,对照组采用氨茶碱治疗,连续治疗3天,监测观察7天如无呼吸暂停发生则停药。结果 :治疗3天后,联合组患儿的PaO2、SaO2水平均高于对照组,PaCO2水平均低于对照组;治疗3天后,联合组患儿的显效52.17%、有效43.48%、无效4.35%,对照组显效32.61%、有效54.35%、无效13.04%,两组比较差异具有统计学意义;联合组治疗3天内呼吸暂停的发生次数低于对照组,联合组辅助呼吸支持应用率低于对照组,联合组患儿的住院时间低于对照组,两组患儿PDA、BDP及ROP发生率差异无统计学意义。结论 :小剂量枸橼酸咖啡因联合氨茶碱治疗早产低体重呼吸暂停患儿的效果优于单用氨茶碱的效果。
        Objective Study on low dose of caffeine citrate combined with aminophylline in treatment of apnea in children with low birth weight effect. Methods From January 2015 to January 2016 were 92 cases of low birth weight premature apnea in children: a prospective randomized study was divided into combined group and control group with 46 cases in each group, combination group with small dose of caffeine citrate combined with aminophylline treatment, the control group using aminophylline treatment, continuous treatment for 3 days, 7 days without monitoring the occurrence of sleep apnea stop the medicine. Results After 3 days of treatment, the combination group with PaO2 and SaO2 levels were higher than the control group, PaCO2 levels were lower than the control group; after 3 days of treatment, the combination group were 4.35%, 43.48%, 52.17% markedly effective invalid, the control group was 32.61%, effective 54.35%, invalid 13.04%, there was significant difference between the two group; combined treatment group within 3 days of the occurrence of apnea times lower than that of the control group, combination group assisted respiratory support application rate was lower than the control group, hospitalization combined group were lower than control group, two groups were PDA, BDP and ROP, there was no significant difference between them. Conclusion Small doses of caffeine citrate combined with aminophylline in treatment of premature apnea in children with low birth weight effect is better than the single effect of aminophylline.
