基于结构方程模型的肺癌手术患者住院费用影响因素分析

摘要
目的:对肺癌手术患者住院费用的影响因素进行分析,为合理控制住院费用上涨提供参考依据。方法:收集样本省三家三级医院2015—2017年的肺癌手术患者的病例资料,采用单因素分析,结构方程模型对患者住院费用的影响因素进行分析,探讨各影响因素之间的相互关系。结果:住院天数、操作的技术难度等是影响住院费用的重要因素,其中住院天数是对住院费用影响最高的因素,其次为操作的技术难度和风险程度。结论:肺癌患者住院费用较高,经济负担较重,建议针对重点影响因素,从疾病诊治、操作难度等多层面采取措施,一方面合理控制医疗费用,减轻患者就医负担,另一方面提示我们应促进医疗机构转变补偿机制,建立合理的价格激励体系,更多的体现医务人员的技术劳务价值。
Objective: To analyze the influencing factors of hospitalization expenses for patients with lung cancer, and to provide references for reasonable control of hospitalization expenses. Methods: The data of patients with lung cancer surgery in 3 tertiary hospitals in a province from 2015 to 2017 were collected. The factors influencing the hospitalization expenses were analyzed by single factor analysis. The structural equation model was used to analyze the relationship between the influencing factors. Results: The number of hospital stays, technical difficulty of operation and other factors were important factors affecting hospitalization expenses.The hospitalization days made the biggest effect on hospitalization expenses, followed by the technical difficulty and risk degree of operation. Conclusion: Patients with lung cancer have higher hospitalization costs and heavier financial burden. It was recommended to take measures on the multi-faceted aspects such as disease diagnosis and technical difficulty of operation for the key influencing factors.On the one hand, reasonable control of medical expenses and reduction of patients' medical burden were demanded. On the other hand,it needed to promote the transformation of compensation mechanisms for medical institutions, establish a reasonable price incentive system, and better reflect the value of medical labor for medical personnel.
