矮小症患儿IGF-1、IGFBP-3的表达水平及其临床意义

摘要
目的:探讨矮小症患儿血清胰岛素样生长因子-1(serum insulin like growth factor-1,IGF-1)、胰岛素样生长因子结合蛋白-3(insulin like growth factor binding protein-3,IGFBP-3)表达水平及其临床意义。方法:将140例矮小症患儿作为观察组,采用精氨酸和可乐定激发检测血清生长激素水平,根据GH峰值分为A、B、C三组;选择健康儿童48例作为对照组,分别检测IGF-1和IGFBP-3的表达水平。结果:A、B、C组患儿IGF-1和IGFBP-3水平均低于对照组(P <0. 05),A组患儿的两种指标水平低于B、C两组(P <0. 05);将IGF-1和IGFBP-3水平低于对照组单侧95%下限者判定为异常,判断完全性生长激素缺乏矮小症患者的敏感度分别为87. 50%和100. 00%,特异度分别为100. 00%和77. 78%,准确度分别为93. 57%和80. 71%。结论:血清IGF-1和IGFBP-3结合对诊断儿童矮小症有重要意义。
Objective: To explore the expression levels of serum insulin like growth factor-1( IGF-1) and insulin like growth factor bindingprotein-3( IGFBP-3) in children with short stature and its clinical significance. Methods: 140 children with short stature were selected as theobservation group,with serum levels of growth hormone detected by arginine and clonidine stimulation. According to the peak of GH,they were di-vided into three groups: Group A,Group B and Group C. 48 healthy children were selected as the control group with the expression levels of IGF-1 and IGFBP-3 detected. Results: The levels of IGF-1 and IGFBP-3 in Group A,B and C were significantly lower than those in the controlgroup( P < 0. 05),and the two indexes in Group A were significantly lower than those in Group B and Group C( P < 0. 05). If one of the IGF-1 and IGFBP-3 levels was lower than 95 % of the lower limit of the control group,the value was abnormal. According to this standard,the sen-sitivity of complete growth hormone deficiency( CGHD) was 87. 50 % and 100 % respectively,the specificity was 100 % and 77. 78 % respec-tively,and the accuracy was 93. 57 % and 80. 71 % respectively. Conclusion: The combination of serum IGF-1 and IGFBP-3 is of great signif-icance in the diagnosis of children with short stature.
