“5.17”爆炸事件烧伤特点与救治

摘要
[目的]回顾性分析西峡"5.17"爆炸烧伤特点与救治,探讨爆炸烧伤的救治策略。[方法]以接诊的13例严重爆炸烧伤伤员为对象,详细记录伤员性别、年龄、烧伤面积、烧伤深度,烧伤合并伤及并发症、呼吸功能的维持、休克期复苏补液、休克期后切痂微粒皮肤移植等信息,分析器官功能支持及90 d综合治疗效果。[结果]休克期平稳度过后第4天开始切痂微粒皮+同种异体皮移植,1周后进行第2轮微粒皮肤移植手术,24 h死亡1例,72 h死亡2例,30 d死亡6例(46.1%),90 d死亡7例(53.8%)。[结论]依靠区域创伤医疗中心的辐射带动作用,及时完成紧急救援期三级救治是西峡特大爆炸事故救治工作的主要模式。
[Objective]To retrospectively analyze the injury features of survived casualtites and treatment strategies in the explosion accident on May 17 2018 in Xixia county.[Methods]A total of 13 survived victims in Xixia"May 17" explosion accident were studied retrospectively. The age, gender, type of injury,burned extent and depth of the patients,resuscitation during shock stage, skin grafting with excision and scab, burns complicated by trauma, complication of burn, were recorded. Data of organ function, 90 d comprehensive treatment effect of the burned victims were analyzed. [Results]Immediately after the explosion, the first aid department of Nanyang Emergency Center carried out the pre hospital care for the wounded on the scene quickly. After meticulous treatment, the mean 24 h mortality was 7.6% and 30 d mortality was 46.1%, 90 d mortality was 53.8%,mainly due to septic shock and multiple organ dysfunction syndrome.[Conclusion]The Level Three treatment with the radiative role of regional trauma centers was the main rescuing mode of the work in Xixia county.
