剖宫产瘢痕妊娠的彩色多普勒超声分型及其在临床治疗中的应用效果

摘要
目的分析剖宫产瘢痕妊娠(CSP)患者临床彩色多普勒超声分型情况及其在临床治疗中应用效果。方法选择医院收治的CSP患者136例作为研究对象,依据患者妊娠物、妊娠物位置以及子宫浆膜层距离不同,分为外生型以及内生型2种,之后依据类型不同合理进行对应治疗方法的选择,最终对治疗效果展开对比。结果本研究的136例CSP患者中,最终发现属于外生型26例以及内生型110例。临床选择UAE(子宫动脉栓塞)+MTX(甲氨蝶呤)进行治疗,最终发现所有患者全部成功完成手术治疗,其中一次治疗成功者132例,二次治疗成功者4例。同外生型CSP患者出血量、住院时间、包块周边血流供应减少时间对比,内生型CSP患者明显减少(P <0. 01)。结论彩色多普勒对于CSP的诊断具有较高应用价值,更可为该病治疗方法的选择提供指导与参考,值得临床推广应用。
Objective To analyze the application of color Doppler ultrasonography in the clinical treatment of cesarean scar pregnancy( SCP). Methods 136 CSP patients were selected and divided into exogenous type and endogenous type according to the pregnancy,the position of the pregnancy and the distance of the uterine serosa. Choose the corresponding treatment methods according to different types,and compare the final treatment effect. Results Of 136 CSP patients,26 were exogenous and 110 were endogenous. Uterine artery embolization( UAE) and methotrexate( MTX) were selected clinically.All patients completed the operation. 132 patients were treated successfully at one time and 4 patients were treated successfully at the second time. The amount of bleeding,hospitalization time and the decrease of blood flow around the mass in endogenous CSP patients were less than or shorter than those in exogenous CSP patients( P < 0. 01). Conclusion Color Doppler has high application value in the diagnosis of CSP,and can provide guidance and reference for the choice of treatment methods of CSP,which is worthy of clinical popularization and application.
