摘要
目的制订不同胎龄新生儿身长胸围指数,为评价新生儿出生时身体的匀称性提供参考数据。方法采用横断面时间段整群抽样实况调查方法,于2013年4月至2015年9月在深圳市两家医院完成了16 887例不同胎龄单胎新生儿体重、身长、顶臀长、头围、胸围现场测量,用以制订不同胎龄新生儿身长胸围指数(BCI)。百分位曲线拟合应用Lambda Mu Sigma(LMS)方法。结果制订了2015年深圳胎龄27~42周单胎新生儿(男、女、性别混合)三分组的BCI均数和3rd~97th百分位曲线。男、女新生儿的BCI数值在胎龄27周最低,随着胎龄增加BCI数值不断增大,胎龄42周BCI数值最大。揭示胎儿宫内生长的胸廓发育伴随胎龄增加不断提高,胎龄42周胸廓发育度最佳。在胎龄27~42周,女性BCI的50th百分位曲线数值明显高于男性,增加值为0.22~1.07;t=3.05~7.87,均P<0.001(胎龄37~41周),差异有统计学意义。结论不同胎龄新生儿胸廓发育度伴随胎龄增加不断提高,胎龄42周胸廓发育度最佳。女性宫内生长胸廓发育度优于男性。
        Objective To create body length-chest circumference index(BCI)of singleton neonates in different gestational age(GA)so as to provide a reference data for evaluating the proportion degree of neonates at birth.Methods By cross-sectional time cluster sampling fact-finding investigation method,a total of 16 887 singleton neonates in different GA were measured including birth weight,length,sitting height,head and chest circumstance from April 2013 to September 2015 in two hospitals in Shenzhen for formulating BCI of neonates in different GA.Percentile curves were smoothed by Lambda Mu Sigma(LMS)method.Results The mean of BCI and 3 rd-97 th percentile curves of singleton neonates(male,female,and unisex)in three groups in 27-42 weeks of GA were created in Shenzhen in 2015.The BCI was lowest at 27 weeks of GA.When GA increased,BCI increased and the highest value appeared at 42 weeks of GA both in male and female,which showed that body chest fullness increased with increasing of GA in uterus.The thorax developed best at 42 weeks of GA.The 50 th percentile curve of female BCI was obvious higher than that of male in 27-42 weeks of GA,and the value added was 0.22-1.07(t value ranged 3.05-7.87,P<0.001)(37-41 weeks of GA)with statistical significance.Conclusion The thorax of neonates in different GA develops with the increasing of GA,and the enrichment degree is highest in 42 weeks.The thorax development of female is superior to that of male.
