摘要
目的:基于舌苔菌群探讨辛开苦降法对有效治疗的上消化道癌前病变的微生态学机制。方法:辛开苦降法治疗上消化道癌前病变患者24例,胃镜及病理诊断进行疗效评价,基于16S r DNA高通量测序分析舌苔菌群的变化。结果:辛开苦降法治疗上消化道癌前病变的有效率为75.0%。与治疗前比较,颗粒链菌属、孪生球菌属和嗜血杆菌属丰度显著上升(P<0.05),依赖杆菌属丰度显著下降(P<0.05)。菌群共生关系显示颗粒链菌属、嗜血杆菌属和孪生球菌属间存在显著正相关(P<0.05),而颗粒链菌属、孪生球菌属、嗜血杆菌属与弯曲杆菌属存在显著负相关(P<0.05)。比较治疗前后舌苔菌群功能预测,辛开苦降法能够显著提高机体无机离子转运与代谢功能(P<0.05)。结论:舌苔菌群可为辛开苦降法有效治疗上消化道癌前病变提供潜在生物标志。
        Objective: To explore the effects of pungent dispersion bitter purgation method(PDBPM) on the bacterial flora of tongue coating in patients with precancerous lesions of upper gastrointestinal(PLUG). Methods: Twenty-four cases of PLUG patients were treated by PDBPM for at least one month. Gastroscope and pathology were used to evaluate the therapeutic effect. The bacterial flora of tongue coating from the ameliorative PLUG patients was detected by 16 S r DNA next high through put sequencing. Results: Seventy-five percent of PLUG patients(18/24) treated with PDBPM were relieved by one or two grades. After PDBPM treatment, the abundance of three genera(i.e. Granulicatella, Gemella, Haemophilus) was significantly increased(P<0.05). However, the abundance of Fretibacterium was significantly decreased(P<0.05). The symbiotic relationship presented a significantly positive correlation among Granulicatella, Gemella and Haemophilus(P<0.05), while Campylobacter significantly correlated negatively to Granulicatella, Gemella, Haemophilus(P<0.05). Based on the predicted function of the bacterial flora of tongue coating, PDBPM treatment could significantly improve the function of inorganic ion transport and metabolism(P<0.05). Conclusion: The bacterial flora of tongue coating may provide potential diagnostic biomarkers of PLUG effectively treated by PDBPM.
