学龄期近视儿童黄斑区视网膜厚度分析

摘要
目的观察学龄期近视儿童黄斑区视网膜厚度变化。方法回顾性病例对照研究。收集自2016年1月至2016年12月进行健康体检的近视儿童98例(187眼),其中低度近视48例(96眼),高度近视50例(91眼)。采用频域OCT测量2组黄斑区视网膜厚度,IOL Master测量2组眼轴长度。采用独立样本t检验比较2组间黄斑区视网膜厚度。不同区域黄斑区视网膜厚度与眼轴长度、屈光度的相关性采用Pearson相关分析。结果两组中均发现黄斑中心凹视网膜厚度最薄;内环区中心凹上方视网膜厚度最厚,颞侧视网膜厚度最薄;外环区鼻侧视网膜厚度最厚,颞侧视网膜最薄。黄斑中心凹区视网膜厚度两组间差异无统计学意义。高度近视组内环区颞侧、外环区颞侧、上方及下方黄斑区视网膜厚度较低度近视组薄,差异有统计学意义。黄斑中心区视网膜厚度(CST)与等效球镜呈负相关,与眼轴长度呈正相关。内环平均视网膜厚度、内环下方视网膜厚度与等效球镜呈正相关,与眼轴长度呈负相关。外环平均视网膜厚度、外环上方、下方、颞侧视网膜厚度与等效球镜呈正相关,与眼轴长度呈负相关。结论学龄期近视儿童,随着近视程度加深,视网膜厚度越薄,且以黄斑中心凹外环区域视网膜变薄为主。
        Objective To observe changes of macular retinal thickness in schoolaged children with myopia.Methods Children from January to December 2016, whose vision in the ophthalmological health examination were recorded, 98 children(187 eyes) were selected, including 48 cases(96 eyes) with low myopia and 50 cases(91 eyes)with high myopia. Binocular macular retinal thickness of all children were carried out by frequency domain optical coherence tomography examination. All eye axis length were measured with IOL Master. Data were compared by independent t test and Pearson correlation analysis. Results In the two groups the central subfield thickness were the most thin and there was no significant difference between the two groups. The retinal thickness of superior inner ring area was the most thick while temporal was the most thin. The retinal thickness of nasal outer ring area was the most thick while temporal was the most thin. The retinal thickness of inner ring area temporal and outer ring area temporal, superior, inferior in high myopia group were thinner than low myopia group. Central subfield thickness had negative correlation with spherical equivalent degree,but had positive correlation with the axial length. Average inner macular thickness and inferior inner ring of macular had positive correlation with spherical equivalent degree, but had negative correlations with the axial length. Conclusions In schoolaged children with an increase in the degree of myopia, retinal thickness is thinner and thinner. The thinning retina is mainly localized in foveal outer ring area.
