手法复位配合石膏固定对踝部骨折患者的治疗效果

摘要
目的:探讨手法复位配合石膏固定对踝部骨折患者的治疗效果。方法:选取2016年1月-2017年1月在本院接受治疗的踝部骨折患者80例,根据不同治疗方式将患者分为观察组和对照组,每组40例。观察组患者采用手法复位配合石膏固定治疗,对照组患者采用手术切开复位联合解剖钢板内固定治疗。比较两组患者的治疗效果、术中出血量、手术时间、住院时间、疼痛消失时间、消肿时间、骨折愈合时间以及踝关节功能。结果:观察组患者的优良率明显高于对照组,差异有统计学意义(P<0.05)。观察组患者的术中出血量明显少于对照组,手术时间、住院时间、疼痛消失时间、消肿时间及骨折愈合时间均短于对照组,差异均有统计学意义(P<0.05)。与治疗前比较,两组患者治疗后3个月的Baied-Jackson踝关节评分、Mazur功能评分均显著升高,差异均有统计学意义(P<0.05);且观察组患者治疗后3个月的Baied-Jackson踝关节评分、Mazur功能评分均显著高于对照组,差异均有统计学意义(P<0.05)。结论:采用手法复位配合石膏固定治疗踝部骨折的患者,可提高治疗效果,减少术中出血量,缩短手术时间及住院时间,可以在临床中推广运用。
Objective:To investigate the therapeutic effect of manual reduction combined with cast immobilization on patients with ankle fracture.Method:Eighty patients with ankle fractures who were treated in our hospital from January 2016 to January 2017 were selected,patients were divided into observation group and control group according to different treatment methods,40 cases in each group.Patients in the observation group were treated with manual reduction and plaster fixation,patients in the control group were treated with open reduction combined with anatomical plate internal fixation.The treatment effect,intraoperative blood loss,operation time,hospitalization time,pain disappearance time,detumescence time,fracture healing time and ankle function were compared between the two groups.Result:The excellent and good rate of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).The amount of intraoperative blood loss in the observation group was significantly less than that in the control group,and the duration of surgery,hospitalization,pain disappearance,detumescence and fracture healing in the observation group were all shorter than those in the control group,with statistically significant differences(P<0.05).Compared with before treatment,the Baied-Jackson ankle score and Mazur function score of the two groups were significantly increased 3 months after treatment,the differences were statistically significant(P<0.05);the Baied-Jackson ankle score and Mazur function score of the observation group were significantly higher than those of the control group 3 months after treatment,with statistically significant differences(P<0.05).Conclusion:Manipulation reduction combined with plaster fixation for the treatment of ankle fracture patients,can improve the treatment effect,reduce the amount of intraoperative bleeding,shorten the duration of surgery and hospital stay,can be used in clinical promotion.
