电视胸腔镜与传统开胸术治疗胸外伤血气胸的效果比较

目的:比较电视胸腔镜与传统开胸术治疗胸外伤血气胸的临床效果。方法:72例胸外伤血气胸患者根据数字表法随机分为两组,对照组(36例)采用常规开胸手术进行治疗,观察组(36例)采用电视胸腔镜手术进行治疗。比较两组患者手术操作时间、术中出血量、术后引流管留置时间、术后引流管引流总量、止痛药物使用时间及术后住院时间;比较两组患者胸膜腔内感染、积血积气、肺不张、肺部感染等术后并发症发生率。结果:两组手术操作时间比较差异无统计学意义(P>0.05);观察组术中出血量、术后引流管留置时间、术后引流管引流总量、止痛药物使用时间及术后住院时间均明显优于对照组,差异有统计学意义(P<0.05);观察组患者胸膜腔内感染、积血积气、肺不张、肺部感染等术后并发症发生率为11.11%,明显低于对照组的30.56%,差异有统计学意义(P<0.05)。结论:电视胸腔镜治疗胸外伤血气胸不但可减少术中出血量、术后引流量,缩短引流管留置时间、止痛药物使用时间、术后住院时间,还可降低术后并发症发生率。
Objective:To compare the clinical efficacy of video-assisted thoracoscopy and traditional thoracotomy in the treatment of hemopneumothorax after thoracic trauma.Method:72 patients with hemopneumothorax after thoracic trauma were randomly divided into two groups according to the number table method,the control group(36 cases) was treated with conventional thoracotomy,and the observation group(36 cases) was treated with video-assisted thoracoscopic surgery.The operation time,intraoperative blood loss,postoperative drainage tube indwelling time,postoperative total drainage tube drainage,analgesic drug use time and postoperative hospitalization time of the two groups were compared.The incidence of postoperative complications including intrapleural infection,hematocele,atelectasis and pulmonary infection of the two groups was compared.Result:There was no significant difference in operation time between the two groups(P>0.05).The amount of intraoperative blood loss,postoperative drainage tube indwelling time,postoperative total drainage of drainage tube,analgesic drug use time and postoperative hospitalization time of the observation group were significantly better than those of the control group,the differences were statistically significant(P<0.05).The incidence of postoperative complications such as intrapleural infection,hematocele,atelectasis and pulmonary infection of the observation group was 11.11%,which was significantly lower than that of the control group(30.56%),the difference was statistically significant(P<0.05).Conclusion:The treatment of video-assisted thoracoscopic for the hemopneumothorax after thoracic trauma can not only reduce the amount of intraoperative blood loss,postoperative drainage flow,shorten the time of drainage tube indwelling,analgesic drug use,postoperative hospital stay,but also reduce the incidence of postoperative complications.
