叶天士《临证指南医案》胃脘痛用药规律探讨

摘要
通过对叶天士《临证指南医案·胃脘痛》篇中的医案用药进行统计分析,发现出现频次较高的药物有温里药、祛湿药、补虚药、理气药、化痰药等7类,共计23种。叶氏治疗胃脘痛注重辨气血、辨虚实、辨寒热、辨温燥。临床常用治法与方剂包括疏肝理气法,用金铃子散;平调寒热法,用泻心汤;清养胃阴法,用叶氏养胃汤;辛甘通阳法,用桂枝汤;活血通络法,用失笑散;温化痰浊法,用大半夏汤。
It is concluded that the herbs that appear frequently include herbs warming the internal,dampness inhibiting herbs,tonics herbs,qi regulating herbs and herbs for resolving phlegm,by studying the stomachache chapter of Clinic Guideline of Medical Records. In the treatment of stomachache,Ye Tian-shi attached importance to the differentiation of cold and heat,qi and blood,deficiency and excess,desiccation and dampness. The commonly used prescriptions and methods in treating stomachache by Ye Tian-shi include relieving the depressed liver by Jinlingzi Powder,regulating cold and heat by Xiexin Decoction,enriching stomach yin by Yeshiyangwei Decoction,diffusing and obstruction-removing gastric yang by Guizhi Decoction,removing blood stasis dredge collaterals by Shixiao Powder,resolving phlegm lowering chaotic by Dabanxia Decoction.
