内镜黏膜下剥离术治疗早期胃癌和癌前病变的临床研究

摘要
目的探讨内镜黏膜下剥离术治疗早期胃癌和癌前病变的临床效果。方法选择2015年3月至2017年2月于我院行早期胃癌及癌前病变治疗的患者40例,随机分为两组各20例。对照组患者实施常规手术治疗,观察组患者实施内镜黏膜下剥离术治疗,比较两组患者的手术指标、术后并发症及复发率。结果观察组患者的手术时间、术中出血量、排气时间、拔除胃管时间和住院时间均显著低于对照组,差异具有统计学意义(P <0.05)。观察组的术后并发症发生率为5.00%,显著低于对照组的35.00%,差异具有统计学意义(P <0.05)。两组术后1年的复发率比较无统计学差异(P>0.05)。结论早期胃癌和癌前病变采用内镜黏膜下剥离术治疗的效果明显,术后并发症发生率和复发率低,值得临床推广应用。
Objective To explore the clinical effect of endoscopic submucosal dissection(ESD) for the treatment of early gastric cancer and precancerous lesions. Methods 40 patients with early gastric cancer and precancerous lesions treated in our hospital from March2015 to February 2017 were selected and randomly divided into two groups, with 20 cases in each group. The control group was treated with regular surgery, while the observation group was treated with ESD. The surgical indictors, postoperative complications and recurrence rate were compared between the two groups. Results The operation time, intraoperative blood loss, exhaust time, gastric tube removal time and hospitalization time of the observation group were significantly lower than those of the control group(P <0.05). The incidence of postoperative complications of the observation group was 5.00%, significantly lower than 35.00% of the control group(P <0.05). No statistical difference was found between the two groups in the recurrence rate at 1 year after surgery(P >0.05). Conclusions Endoscopic submucosal dissection(ESD) for the treatment of early gastric cancer and precancerous lesions has obvious efficacy, and low incidence of postoperative complications and recurrence rate, which is worthy of clinical promotion and application.
