不同深度针刺地仓、颊车穴对周围性面瘫患者生活质量及面神经功能的影响

摘要
目的比较不同深度针刺地仓、颊车穴治疗周围性面瘫的临床疗效。方法将100例周围性面瘫患者随机分成对照组和治疗组各50例。在相同配穴针刺治疗基础上,对照组采用常规手法针刺地仓、颊车穴,治疗组根据不同患者的体型地仓穴向下斜刺约4~6 cm、颊车穴向地仓穴方向深刺约3. 3 cm。两组均每周治疗3次,30天后进行疗效评估,并于治疗前后比较两组患者中医症状评分、生活质量评分、Sunnybrook面神经评分。结果治疗组临床疗效总有效率为88. 0%,对照组为66. 0%,治疗组疗效优于对照组(P <0. 05)。治疗后两组患者各中医症状评分均明显下降(P <0. 05),且治疗组口角歪斜、眼裂闭合评分均较对照组明显下降(P <0. 05)。治疗后两组患者生活质量各项评分、Sunnybrook面神经评分均明显提升(P <0. 05),且治疗组生理功能、生理职能、总体健康、精神健康评分及Sunnybrook面神经评分均较对照组明显提升(P <0. 05)。结论深刺地仓、颊车穴能够有效改善周围性面瘫患者症状、提高生活质量、恢复面神经功能,其较常规浅刺疗效更佳。
Objective To compare the effects of different depths of acupuncture at acupoint Dicang(ST 4) and acupoint Jiache(ST 6) in the treatment of peripheral facial paralysis. Methods A total of 100 patients with peripheral facial paralysis were randomly divided into control group and observation group,with 50 patients in each group. The control group used conventional techniques to acupuncture at Dicang(ST 4) and Jiache(ST 6). According to the body shape of different patients,the observation group were deeply penetrated 4 ~ 6 cm at Dicang(ST 4)and pricked 3. 3 cm into the direction of the Jiache(ST 6). Both groups received treatment three times a week. After30 days of treatment,the curative effect was evaluated to observe the improvement of traditional Chinese medicine(TCM) symptom score,quality of life improvement and Sunnybrook facial nerve function score. Results The total effective rate of the observation group was 88. 0%,which was significantly higher than that in the control group(66. 0%,P < 0. 05). After treatment,the TCM symptom scores in both groups were significantly decreased(P <0. 05). The scores of distortion of commissure and cleft eye closure in the treatment group were significantly lower than those in the control group(P < 0. 05). The quality of life scores and Sunnybrook facial nerve function score of patients were significantly increased compared with that of treatment group than that of control group(P < 0. 05). The physiological function,physiological role,overall health,mental health score and Sunnybrook facial nerve score of the treatment group were significantly improved compared with the control group(P < 0. 05). Conclusion The use of acupuncture of Dicang(ST 4) and Jiache(ST 6) to treat peripheral facial paralysis is effective,which can effectively improve the patient's symptoms,the quality of life of patients,and restore facial nerve function.
