社区健康管理在城市社区慢性病管理中的应用及对慢性病控制率影响分析

摘要
目的在城市社区慢性病管理中应用社区健康管理,分析社区健康管理对慢性病管理效果。方法选择某市某社区居民共计200例,选择阶段在2016年2月至2018年2月,分不同的组别行差异性管理方式。研究组予以社区健康管理,对照组予以常规健康管理。对比分析组间的慢性病控制率、疾病知识掌握率、血糖和血脂达标率。结果研究组的慢性病控制率为81.00%、慢性病疾病知识掌握率为80.00%、血糖和血脂达标率为79.00%。各项指标均高于对照组(P<0.05)。结论社区健康管理在城市社区慢性病管理中的应用价值显著,可以显著提高慢性病控制效果。
Objective To analyze the impact of community health management on the incidence of chronic diseases by applying community health management to the management of chronic diseases in urban communities. Methods A total of 200 urban community personnel were selected during the selection period from February 2016 to February 2018, they were divided into different groups and managed differently. The study group was given community health management, while the control group was given routine health management. The control rate of chronic diseases, the mastery rate of disease knowledge, the rate of blood sugar and blood lipid reaching the standard were compared and analyzed. Results The control rate of chronic diseases(81.00%), the mastery rate of disease knowledge(80.00%), the rate of blood sugar and lipid reaching the standard(79.00%) in the study group were higher than those in the control group(P<0.05).Conclusion The application value of community health management in urban community chronic disease management is significant, which can significantly reduce the incidence of chronic diseases, and the feasibility of application is very high.
