正畸力作用下牙髓组织神经生长因子的表达及其与疼痛的关系研究

摘要
目的探讨正畸力作用下牙髓组织神经生长因子(NGF)的表达及其与疼痛的关系。方法选取口腔正畸患者26例,依据正畸计划均需拔除上颌双侧第一前磨牙(52颗牙)并进行固定矫治。患者在安装矫治器前均拔除左上颌第一前磨牙(对照组,26颗牙),分别在安装矫治器后1、3、7、15d(7、7、6、6例)拔除右上颌第一前磨牙(观察组,26颗牙)。观察并比较两组牙齿疼痛程度、牙髓组织NGF染色阳性细胞着色面积密度(AD)、平均光密度值(AOD),分析牙髓组织NGF表达强度与牙齿疼痛程度的相关性。结果两组牙齿疼痛程度、牙髓组织NGF染色阳性细胞AD与AOD比较差异均有统计学意义(均P<0.05),观察组安装矫治器后1、3d牙齿疼痛程度、牙髓组织NGF染色阳性细胞AD与AOD均高于对照组(均P<0.05),观察组安装矫治器后3d的指标均高于安装矫治器后1d(均P<0.05)。正畸力作用下牙髓组织NGF表达强度与牙齿疼痛程度呈高度正相关(r=0.867,P<0.05)。结论正畸力作用下牙髓组织NGF的表达上调,且其表达强度与牙齿疼痛程度呈高度正相关。
        Objective To investigate the expression of nerve growth factor(NGF) in dental pulp under orthodontic force and its relationship with pain. Methods Twenty six oral orthodontic patients were enrolled in the study. According to the orthodontic plan, 52 first premolars on both sides of the upper closure were extracted, and fixation and correction were followed. Twenty six left superior first premolars were extracted before the orthodontic device was installed(control group), and 26 right superior first premolars were extracted1, 3, 7 and 15 d after the orthodontic device was installed(study group, n=7, 7, 6 and 6). The degree of tooth pain, the area density(AD) and the average optical density(AOD) of NGF positive cells in the pulp tissue were observed and compared between two groups. The correlation of NGF expression in the pulp tissue with the degree of tooth pain was analyzed. Results The degree of tooth pain 1 and 3 d after device installed, and the AD and AOD of NGF positive cells in the pulp of study group were higher than those of control group(P<0.05). The indexes of study group were all higher at 3 d than those at 1 d after the device installed(P<0.05). The expression intensity of NGF in pulp tissue was positively correlated with the degree of tooth pain(r=0.867, P<0.05). Conclusion Under the orthodontic force, NGF expression in pulp tissue is up-regulated, which is positively correlated with the pain of teeth.
