探讨RT-3DE评估右心衰竭患者右室收缩功能的临床价值

摘要
目的:探讨实时三维超声心动图(Real Time Three-Dimensional Echocardiography,RT-3DE)评估右心衰竭(Right-Sided Heart Failure,RHF)患者右室收缩功能的临床价值。方法:选取我院确诊的RHF患者50例,将RHF早期阶段患者25例分为Ⅰ组,有明显症状和体征的RHF患者25例分为Ⅱ组,同时选取25例同期健康志愿者分为Ⅲ组。常规测量三组游离壁厚度(RV-FW)、右室基底部左右径(RV-D)、右心房上下径(RA-D2)、右心房左右径(RA-D1)、右室面积变化分数(Right Ventricular Fractional Area Change,RVFAC)、右室心肌做功指数(RIMP)、三尖瓣环收缩期峰值速度(S')及三尖瓣环收缩期位移(Tricuspid Annular Plane Systolic Excusion,TAPSE),RT-3DE测量右室收缩末期容积(Right Ventricular end-systolic Volume,RVESV)、右室舒张末期容积(Right Ventricular edn-diastolic volume,RVEDV)、右室射血分数(Right Ventricular Ejection Fraction,RVEF)及右室每搏输出量(Right Ventricular Stroke Volume,RVSV)。观察三组各指标间差异。结果:三组RA-D1、RA-D2、RVFAC、TAPSE、RVEF、RVESV、RVEDV、RVSV差异均具有统计学意义(P <0.05);RV-FW、RV-D、S'在Ⅰ组与Ⅱ组,Ⅰ组与Ⅲ组间差异具有统计学意义(P <0.05);RIMP仅在Ⅰ组与Ⅱ组间差异具有统计学意义(P <0.05)。结论:RT-3DE评价RHF患者右室收缩功能具有较高灵敏度和准确性,可快速确诊疾病有助于临床诊断和治疗,值得推广。
Objective: To evaluate the clinical value of RT-3 DE in assessing right ventricular systolic function in patients with RHF.Methods: Fifty patients with RHF diagnosed in our hospital were selected. Twenty-five patients with early stage RHF were divided into group I. Twenty-five RHF patients with obvious signs and symptoms were divided into group II and 25 healthy volunteers of the same period were divided into Ⅲ groups. Free wall thickness, right ventricular base diameter, right atrium up and down diameter, right atrium lateral diameter, right ventricular area change score, Right Ventricular Myocardial Work Index, Tricuspid Annulus Systolic Peak Velocity,and Tricuspid Annular Systolic Shift in three groups were given routine measurement. Right Ventricular End-systolic Volume, RVEDV,RVEF and RVSV were given measurement by RT-3 DE. The differences among three groups of indicators were observed. Results: The differences of RA-D1, RA-D2, RFAFC, TAPSE, RVEF, RVESV, RVEDV, and RVSV among three groups were statistically significant(P<0.05); There was a significant difference in RV-FW, RV-D, and S' between group I and II or between group I and group III(P<0.05).There was a statistically significant difference in RZMP between group I and group II(P<0.05). Conclusion: RT-3 DE has a high sensitivity and accuracy in the evaluation of right ventricular systolic function in patients with RHF. Rapid diagnosis of the disease may be helpful in clinical diagnosis and treatment and is worthy of promotion.
