支气管镜肺泡灌洗治疗老年脑卒中并重度肺炎患者疗效观察

摘要
目的观察支气管镜肺泡灌洗治疗老年脑卒中并重度肺炎患者的临床疗效。方法将100例老年脑卒中并重度肺炎患者随机等分为试验组和对照组,每组50例。试验组患者给予支气管镜肺泡灌洗,对照组患者应用内科常规抗感染治疗。比较2组患者治疗前后血管紧张素Ⅱ(AngⅡ)水平变化及治疗后血气分析。结果 2组患者治疗前AngⅡ水平差异无统计学意义(P>0.05),治疗后2组患者AngⅡ水平均低于治疗前,且试验组低于对照组(P<0.05)。试验组患者治疗后的PaO_2、PaCO_2、pH等指标均优于对照组(P<0.05)。结论临床针对老年脑卒中并重度肺炎患者应用支气管镜肺泡灌洗治疗具有较好的临床疗效。
Objective To observe the clinical curative effect of bronchoscopy alveolar lavage in treatment of elderly stroke and severe pneumonia. Methods 100 patients with severe stroke and severe pneumonia were randomly divided into experimental group and control group,50 cases in each group. The patients in experimental group were treated with bronchoscopy alveolar lavage,and the patients in control group were treated with conventional anti-infective medicine. The changes of angiotensin Ⅱ(Ang Ⅱ) before and after treatment and blood gas analysis after treatment were compared between two groups. Results There was no significant difference in Ang Ⅱ level between the two groups before treatment(P > 0. 05). After treatment,the levels of AngⅡ were lower in two groups than before treatment,and experimental group was lower than control group(P < 0. 05). The PaO_2,PaCO_2 and pH in experimental group were better than those in control group after treatment(P <0. 05). Conclusion Clinical application of bronchial lumen irrigation in patients with severe stroke and severe pneumonia has a good clinical effect.
